Parkview Physicians Group is currently seeking experienced Board Eligible / Board Certified Family Medicine or Internal Medicine physicians to join our Population Health team in Fort Wayne, Indiana.

Specifics of The Role

Full time and part time opportunities available Hours are Monday-Friday from 8am – 5pm Involves travel by conducting daily rounding on Parkview patients in high performing network skilled nursing facilities Nurse practitioners cover call; physician would need to be available as a backup by phone

The Team

• Group consists of 1 physician & 5 advanced practice providers that are dedicated to providing quality continuation of care to patients that are discharged from the hospital
Comprehensive care team and clinical support model comprised of medical assistants and registered nurses
