Evaluates, plans and performs treatments to alleviate physical, cognitive/perceptual, psychosocial and or developmental dysfunction using purposeful activities and restorative therapeutic techniques. Participates in departmental activities to assure smooth operations and facilitate program enhancement.
Position Summary:
• Administers therapy evaluations and develops treatment plan for physically, cognitively, developmentally, and/or functionally disabled patients.
• Utilizes individualized treatment plans designed to restore, remediate or compensate for losses in physical, perceptual, sensory neuromuscular, sensory integrative, cognitive and psychosocial skills. Fabricates orthotics and adaptive equipment, employs physical agent modalities according to training/education and current competency.
• Educates patient, family, and other caregivers in therapy programs and goals.
• Participates in patient care meetings, collaborates with team members, assists discharge planning.
• Delegates caseload to therapy assistants, overseeing quality of care and supervising performance.
• Participates in QI activities and program development.
• Documents: evaluation and care in medical record; therapy billing; discipline statistics.
Major Responsibilities:
Performs and documents occupational therapy evaluation according to policy and procedure considering the developmental and psychosocial needs of Geriatric patients.
Develops and documents an individualized patient treatment plan. Treatment plan utilizes department protocols and is appropriate to the developmental and psychosocial needs of Geriatric patients.
Demonstrates competency in clinical skills as outlined in the attached occupational therapist skills list.
Performs and documents patient reassessment according to department policy and procedure and considering the developmental and psychosocial needs of Geriatric patients.
Develops and documents a patient education plan. Education plan considered the patient’s readiness to learn, developmental and cognitive abilities, and appropriate education methods.
Documentation is in accordance with Hospital and Department policies and procedures.
Education plan demonstrates adjustment for Geriatric patients.
Demonstrates respect for the patient’s right to participate in treatment decisions evidenced through documentation of patient participation in treatment planning and observation by supervisor.
Demonstrates respect for the patient’s right to privacy. Competency is measured by supervisor’s observation of therapist providing privacy during evaluation and treatment.
Develops and documents an individualized discharge plan. Participates in interdisciplinary discharge planning.
Documentation is legible and in accordance with department/hospital policy and procedure.
Completes errorless charging forms for patient services on a daily basis.
Demonstrates adherence to Hospital policy and procedure on Incident Reporting and Patient Complaints.
Participates in professional development by attending at least 8 hours of continuing education annually.
Participates in the professional development of peers and other members of the department and hospital health care team by providing at least 2 hours of education annually.
Provides supervision to therapy assistants and aides. Delegates patient care that the assistant/aide has been deemed competent to deliver. Provides input to Rehabilitation Director regarding skills of assistants/aides.
Provides supervision for students and volunteers. Ensures that activities of students/volunteers are within department guidelines.
Follows hospital and department policies and procedures.
Maintains current knowledge of department activities demonstrated by attendance at staff meetings and/or reading and initialing meeting minutes, memo's and notices.
Consistently communicates in a calm, professional manner.
Meets established productivity standards.
Demonstrates use of Quality Improvement in daily operations.
Complies with all health and safety regulations and requirements.
Respects diverse views and approaches, and contributes in maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
Performs other similar and related duties as required or directed.

Position Qualifications:
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
• Ability to read, speak and write English.
• Ability to talk and hear for discussion with patients and co-workers.
• Visual acuity of at least 20/40 corrected near and far. Color vision for perceiving changes in skin color.
• Demonstrated ability to provide competency requirements of job description.
• Ability to lift, carry, push, pull, stoop, and reach for equipment, supplies and patient up to 50 pounds without assistance; over 50 pounds with mechanical or other personnel assistance.
MINIMUM QUALIFICATIONS
• Bachelor’s Degree in Occupational Therapy from an accredited OT school.
• Current license to practice as Occupational Therapist in Massachusetts
• AOTCB certification
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
