Physical Therapist (PT) – Boswell Transitional Care of Cascadia –

Boswell Transitional Care is looking for a Full Time Physical Therapist (PT) to join our team to be a FORCE for GOOD in Sun City, Arizona! Join us in our 115-bed facility with TWO therapy gyms and an all IN HOUSE therapy team.

Position Summary:

In addition to the duties listed below, this position will allow the clinical ownership and flexibility that Physical Therapy is built on. There is no “one way” to achieve anything and we strive to ensure our team members have the freedom to construct a program that truly puts our patients at the center while also dedicating energy to ensuring each team member feels valued. You will “own” your cases and be encouraged to bring new ideas to support your patients and the departments success. We staff 2+ rehab techs that support you on the floor to make sure you have what you need to provide amazing care.

Essential Functions

• Establishes and administers a treatment plan with specific therapy goals determined according to the patient's capacity and tolerance under the direction of the physician.
• Assesses patient needs; plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
• Sets realistic and achievable goals for patients/residents.
• Documents and records the patients’/residents’ condition and progress per state and federal regulations.
• Educates patients and families in an appropriate therapy method.
• Optimizes the functionality and skills of patients by providing, directing and/or overseeing the screening of all residents.
• Communicates patient status and needs to the patient, family, and other involved professionals.
• Ensures punctuality and regular attendance for assigned shifts.

Other Functions

• Adhere to Minimum Data Set (MDS) procedures and effective care planning.
• Provides consultation and clinical supervision to PTA’s working in the facility.
• Other duties as assigned.

Knowledge/Skills/Abilities

• Ability to maintain confidentiality.
• Knowledge of HIPAA.
• General knowledge and understanding of managed care.
• Knowledge of state and federal rules relating to therapy delivery and documentation in skilled nursing facilities.

Qualifications

Education:

Bachelor’s, Master’s or Doctorate Degree from a college or university with an accredited Physical Therapy program.

Licenses/Certification:

Current Physical Therapist license in the state employed.

Current CPR certification.

Level 1 Fingerprint Clearance Card

Experience:

Six months experience in a long-term care environment preferred.
