JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Work primarily with youth with co-occurring intellectual disorder, serious mental illness or emotional disorders, Autism Spectrum Disorder and their families, with the goal to maintain the youth in the least restrictive setting with regards to home, school and community programs. Work closely with youth and their families in providing the support, training and assistance required for a stable and healthy family and community life.

PHYSICAL DEMANDS:
Minimal in nature
QUALIFICATIONS:
Bachelor’s degree in human services field from an accredited college/university. One to two years’ experience in the behavioral health field and experience working with children and adolescents preferred. Driver's License and Vehicle Required.
SPECIFIC DUTIES:
- Provide a comprehensive strengths-based assessment for each assigned youth with complex needs and their family caregivers
- Develop with each youth and family, a family action plan addressing the needs for treatment, skill development, social and emotional development and social service needs
- Ensure that the principles developed by Pennsylvania Child and Adolescent Service System Program (CASSP) are followed in providing services for children and youth, and their families
- Coordinate psychiatric services as indicated for assigned children and youth
- Coordinate more structured collaboration across teams and systems
- Provide comprehensive crisis prevention planning
- Facilitate family team meetings and encourage family stories
- Assist the youth and family in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation, education.
- Coordinate continuity of care for each assigned child/youth.
- Provide Wrap-around informed approach which includes children and youth, and family voice and choice.
- Provide home, community and school visits to each assigned child/youth and family
- Monitor service delivery by maintaining contact with assigned child/youth, families and other service providers involved with them.
- Complete face-to-face contact with each assigned child/youth in accordance with CBH and DBH guidelines.
- Actively participate in discharge planning for assigned child/youth, including if a child/youth is hospitalized
- Advocate for assigned children and youth, and families as needed
- Attend all meetings related to assigned child/youth, as well as other assigned meetings
- Respond to all emergencies and crises of assigned child/youth and mobilize available resources to assist the child/youth and family in crisis
- Provide 24-hour crisis coverage for eligible child/youth as assigned and respond to each call within 15 minutes.
- Staff will participate in Family Driven Teaming (FDT) training and consultation.
- Be aware of family’s financial liabilities and coordinate any financial changes with PATH’s Billing
Department, and assist families in securing third-party resources when indicated.
- Meet agency productivity standards for your position.
- Complete all paperwork in a timely, comprehensive, legible manner and in accordance with PATH policies and procedures.
