Summary:
The Street Outreach Case Manager, as part of the Tumaini Center Homeless Recovery Services team and in collaboration with the person served, is responsible for providing assessment, case plan development, housing placement and linkage to benefits. This position is a community-based position, which involves intensive outreach services. The Street Outreach Case Manager provides services to an assigned caseload under the supervision of the PATH Outreach Supervisor.
Responsibilities:
Primary Job Duties:
- Case Management
- Provides street outreach to engage consumers who are defined as unsheltered.
- Provide follow-up services to known locations as identified by the person served.
- Performs thorough intake and mental health assessments on participants.
- Participates in team meetings and presents information obtained through the screening and assessment process.
- Develops service plans for participants based on intake and housing needs.
- Directly engages landlords/property managers.
- Refer participants to the Coordinated Assessment Model for housing placement.
- Assist participants through the Housing Choice Voucher process when scoring for Rapid Rehousing, including facilitating housing placement, and attending lease signings.
- Assist participants in understanding and abiding by their lease obligations.
- Carries out eviction prevention plans when needed.
- Provides intensive support services until participant is successfully housed.
- Assist participants in obtaining necessary household items.
- Documents services provided in a timely and accurate manner.
- Case Documentation
- Completes progress notes (which includes HMIS service transactions and HMIS case notes) according to agency policy.
- Uploads all required housing documents into HMIS.
- Maintains copies of all housing related documents within the participant’s record.
- Completes Assessments in a thorough and timely manner.
- Completes a Case Plan that addresses the needs and barriers identified during assessment and establishes measurable goals and objectives within a specified time frame.
- Completes and makes corrections to reports within time frames requested by supervisor.
- Consultation-
- Receives instructions and assignments, providing weekly caseload reports and makes recommendations to the professional team.
- Follows through with assigned tasks.
- Participates in supervisory meetings and team meetings.
- Accessing Community Resources
- Identifies resources within the community and assists clients to access resources such as security deposit, furniture, and other household items.
- Develops and maintains a working knowledge and relationship with providers of resources for clients.
- Apprises Administrative staff of service gaps that affect clients’ functioning within the community.
Candidate Requirements:
Education: BA/BS/BSW in social work, human services ore related discipline
Minimum Required Experience: 2+ years’ experience working with homeless individuals with mental illness required. Previous Street Outreach experience desired, although related experience will be considered.
