JOB TITLE: Residential Nurse
DEPARTMENT: Residential Services
SUPERVISOR’S TITLE: Residential SGL Director/Head Nurse
POSITIONS SUPERVISED: None
JOB STATUS: Full-time; Non-Exempt
SCHEDULE: 1st Shift, Mon-Fri (8am-4pm)
Summary:
Provide nursing support for individuals in residential settings, including monitoring health concerns, consulting on plans of care, training staff, reviewing medication records, and supporting care plan implementation. Responsible for maintaining confidentiality, promoting health and safety, and coordinating nursing-related follow-up and reporting.
Key Responsibilities:
- Visit SGL residents, when appropriate, for minor illness, injuries, or emergencies.
- Consult on the health aspects of the individual’s plan of care.
- Control communicable diseases and infections as needed.
- Attend annual conferences of all group home residents.
- Train SGL, Habilitation, and Work Services staff in detecting signs of illness, seizures, diabetes, insulin shots, G-tube feeding, first aid, and administering drugs according to state regulations.
- Review medication records and audits, appointments, labs, and follow-ups monthly and as needed.
- Complete end-of-month health care summaries and Healthcare Reports at the beginning of each month.
- Review Care Plans and Adaptive Devices yearly or as needed based on new diagnosis or necessity.
- Teach “Living in the Community” Medication curriculum to agency staff monthly as needed.
- Attend staff meetings, nurse/manager training, and monthly medication destruction.
Qualifications & Skills:
- Ability to communicate verbally and in writing with service recipients, co-workers, and the general public.
- Strong documentation, record, review, and follow-up skills.
- Ability to train and support staff on health-related procedures.
Physical & Work Requirements:
- Supply proof of professional liability insurance.
- Supply proof of valid driver’s license.
- Supply proof of vehicle liability insurance.
- Ability to respond to resident health needs in residential settings and communicate as needed with staff.
Certifications & Training Requirements:
- Maintain valid nursing license registration.
- Maintain current CPR, First Aid, and Preventing Disease Transmission.
- Complete Certified Restraint Training (CRT)
- Complete Medication Administration training (Core A and B)
- Participate in in-service training designed to maintain and improve job skills.
Additional Information:
The Residential Nurse works under the direct supervision of the Residential SGL Director/Head Nurse, who may add to or elaborate on duties as deemed appropriate.

Education & Experience Requirements:
- Must be a State licensed LPN in the State of Indiana supervised by an RN.
- One year of experience working with individuals with developmental disabilities
