Job Description
This role is part time, 20 hours per week.
To ensure patients and their families receive timely access to AMOREM services and support by responding with a sense of urgency to all referrals, completing an evaluation of services and assessment of patient’s needs, coordinating care, completing admission consent forms and creating an individualized plan of care tailored to the patient/family needs.
Requirements:ESSENTIAL DUTIES & RESPONSIBILITIES:
· Respond to all referrals with a sense of urgency per AMOREM timeliness of services
· Provide education regarding hospice and palliative care services
· Evaluate the physical, financial, psychological, spiritual and pre-bereavement needs of the patient/family in coordination with interdisciplinary group and admit to the appropriate program
· Determine patient’s eligibility for hospice care/palliative care with AMOREM providers
· If patient is not appropriate, recommend other services available with referrals to other community agencies as appropriate
· Admit patients, obtain informed consent for service, inform patient/family or rights and responsibilities and complete related documentation in accordance with AMOREM policies and procedures
· Communicate patient/family information and needs to the interdisciplinary group via report
· Develop an individualized plan of care with goals and interventions defined by patient/family in collaboration with the hospice interdisciplinary group
· Complete documentation within hours designated in policy
· Coordinate same day phone calls to new admissions and/or request same day visits
· Complete medication recommendation with AMOREM provider and enter medications into Suncoast
· Coordinate durable medical equipment and pharmacy, as needed, for all patients upon admission
· Rotate 24-hour on-call services and provide duty of a primary nurse as needed and as directed by supervisor
· Included in Access & Enrollment Specialist minimal staff days
· Provide report on newly admitted patients to the assigned IDG
· Other duties as assigned
KNOWLEDGE, SKILLS & ABILITIES:
· Knowledge of and commitment to the hospice philosophy of care
· Familiarity with a variety of clinical concepts, practices and procedures
· Exceptional communication skills
· Strong writing skills, organizational and analytical skills
· Must be flexible, adaptable to change, and able to apply problem-solving skills and critical thinking skills
· Emotional resilience, a calm temperament and the ability to work well under pressure
· Knowledge of Medicare/Medicaid rules/regulations necessary to understand criteria for determining patient eligibility for admission
· Willingness to work with patients/families regardless of race, color, creed, age or sexual orientation. Must respect their beliefs and philosophy with an attitude of openness and receptiveness
· Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair
· Ability to work in an environment containing distractions and interruptions such as noise and telephone calls
· Ability to handle varying and unpredictable situations such as emergencies or crisis situations
MINIMUM QUALIFICATIONS:
Education:
· Current NC license as a registered nurse
Experience:
· Minimum 2-3 years of nursing experience required
· Active patient contact or completion of a nursing refresher course within the past 3 years
Licensures & Certifications: (if applicable):
· CPR Certification
PHYSICAL DEMANDS:
· Visual acuity to read written documentation and computer tasks
· Ability to sit extended periods of time and perform administrative tasks
· Ability to stand, bend and lift an average of 30 pounds
· Manual dexterity with the ability to manipulate office equipment and keyboard for extended periods of time
· Ability to walk, squat, kneel, climb and balance

· Must have an automobile with required insurance coverage and state driver’s license
