Job purpose

Ryan White Medical Case Manager will provide comprehensive individualized social services to people living with HIV/AIDS. It is the responsibility of the HIV Case Manager to provide a wide spectrum of services as a part of a primary care team; help to improve the health and well-being of our primary care clients and promote team functioning; conducted targeted outreach; collaborate with outside service providers and make appropriate internal/external referrals.

Duties and responsibilities

  • Link clients to appropriate services that promote positive health outcomes, treatment adherence, and greater self-sufficiency.
  • Help retain clients in care.
  • Links clients to legal, housing and transportation services.
  • Links clients to mental health, substance use and pain management treatments.
  • Assists clients in navigating the requirements of medical insurance, including enrollment in state Medicaid by becoming a Certified Application Counselor (CAC).
  • Enrolls clients in AIDS Drug Assistance Program
  • Assist with and follow-up on the successful completion of health maintenance items (e.g. lab testing, annual visits) and chronic disease management (e.g. routine diabetic or asthmatic care)
  • Tracks client follow up and care plan outcomes
  • Documents client progress, referrals, encounters and services in the Epic EHR and CAREware.
  • Maintains documentation of services to allow compliance with required reporting for Ryan White funding.
  • Maintains strict adherence to all document deadlines including report deadlines and timely completeness of documentation
  • Assists with scheduling and development of Consumer Board and Advisory meetings.
  • Participates in regular case conferences with the HIV team.

Qualifications

  • Associates Degree in a health-related field is required. A high school diploma/GED with relevant years of experience may be considered in lieu of degree requirement. Bachelor’s degree preferred, a valid CT driver’s license and/or access to reliable transportation is required.
  • Experience in case management, preferably with people living with HIV/AIDS; experience using the Epic electronic health record highly preferred.
  • The successful candidate will have excellent computer skills including word processing and data entry required and the ability to work independently. Bilingual in English and Spanish is highly desirable.
