Job Description
The Post-Permanency Case Manager provides comprehensive case management, advocacy, and supportive services to birth families, resource parents, and youth who have connected through foster care, adoption, or guardianship. This role involves direct youth mentorship, family support, training facilitation, and legal documentation.
Essential Duties and Responsibilities Case Management & Family Support
- Offer assessment, planning, counseling/therapy, referrals, and advocacy.
- Help families access and navigate school systems, medical providers, and community resources.
- Lead individual meetings with youth through the MYLIFE service to help them process their foster care and adoption journeys.
- Build and maintain professional relationships within the agency and the broader community.
Group Facilitation & Training
- Run support groups for children and parents.
- Plan, design, and present educational programs and training sessions for foster and adoptive parents.
Legal & Administrative Compliance
- Maintain timely records and documentation matching agency, state accreditation, and policy requirements.
- Complete necessary court updates and official court reports.
- Present professional testimony at court hearings when required.
- Participate regularly in peer reviews, individual supervision, and clinical consultations.
- Maintain strict agency standards for confidentiality, quality, and productivity.
Position Requirements & Schedule
- Based in Wilmington, DE, with statewide travel required as needed.
- Must be available to work after 5:00 PM at least two days per week to meet with youth and families.
Education and Experience
- Bachelor's Degree in Social Work, Human Services, or a related field (Required).
- Previous experience in the foster care, adoption, or human services field (Preferred).

- Ability to respond appropriately to diverse cultural populations.
- Strong verbal and written communication skills.
- Excellent time-management and organizational abilities.
- Strong engagement and relationship-building capabilities.
- Must possess a valid driver’s license meeting agency insurance standards and have access to reliable transportation.
