Seeking Qualified Candidates for our Plymouth Location.

Personalized Nursing LIGHT House is a premier behavioral health care provider with 34 years of specialized addiction treatment service serving the adult population. We have locations in Plymouth, Canton, Brighton, & Madison Heights. Our mission is to be an innovative and effective behavioral healthcare corporation (utilizing the LIGHT model of care), that strives for excellence clinically as well as administratively. Specifically, our intent is to increase client well-being, decrease addiction severity, improve the quality of clients' mental and physical heath, and contribute positively to society. All services will integrate substance use disorder, mental health, and primary health care treatment in cohesive and client-centered programs.

POSITION SUMMARY:

We are seeking responsible and compassionate Case Managers to help locate applicable care and resources for our clients and to transport clients to/from approved resource providers. The ideal candidates will take initiative in instituting case management services and communicating with clients in effective ways. Experience with persons with substance use disorder and/or co-occurring disorders is preferred.

REQUIREMENTS:

  • Bachelor's degree in a related human services field (social work, psychology, criminal justice, etc.). CAADC, CADC or Development Plan from MCBAP.
  • Driver's License and a clean driving record. Applicants must be comfortable driving clients in their personal vehicle.
  • Knowledge of Microsoft Office (Word, Excel, PowerPoint)
  • Be familiar and comfortable with the culture and characteristics of persons with Substance Use Disorders and mental health disorders. Provide a welcoming and accepting environment in which this population and their families can receive services.
  • Be able to multi-task
  • Be confidential and a good listener
  • Have effective communication skills, both written and verbal
