Location: Cochran, GALicensed Mental Health Counselor (LMHC) / Licensed Clinical Social Worker (LCSW)/Licensed Psychologist JOB SUMMARY
Psych Health Associates is seeking a qualified Georgia-licensed LMHC, LCSW or Psychologist to provide high-quality behavioral health services to patients residing in post-acute care facilities. The clinician will travel to multiple facilities each day to conduct patient rounds, communicate with patients and facility staff, and provide comprehensive assessment, diagnosis, treatment, consultation, education, and follow-up care.
The clinician will collaborate closely with providers within the Psychology Division and interdisciplinary facility teams to ensure coordinated, patient-centered care.

- Conduct patient evaluations, obtain clinical histories, and document findings accurately and thoroughly.
- Maintain timely and accurate clinical documentation in accordance with established charting and regulatory requirements.
- Collaborate with physicians and other healthcare professionals to develop comprehensive, individualized treatment plans.
- Submit patient treatment plans, goals, and recommendations for periodic review by the supervising physician, as appropriate.
- Provide patient education regarding health, wellness, coping strategies, disease prevention, and other relevant areas of care.
- Coordinate referrals to specialists and other healthcare providers when clinically indicated.
- Complete required documentation and reporting related to deaths, communicable diseases, and other reportable events in accordance with applicable laws and regulations.
- Collaborate with Psychology Division providers and facility staff to support continuity and quality of care.
- Perform other duties and responsibilities as assigned.
- Must hold and maintain a current Georgia professional license in good standing.
- Must be a Licensed Mental Health Counselor (LMHC) Licensed Clinical Social Worker (LCSW) or Licensed Psychologist authorized to practice in Georgia.
- Current national certification or board certification, when applicable to the clinician's professional discipline.
- Previous experience providing behavioral health services in post-acute, skilled nursing, assisted living, or similar healthcare settings is preferred.
- Strong written, verbal, interpersonal, and electronic communication skills.
- Demonstrated initiative, sound clinical judgment, creativity, and problem-solving ability.
- Ability to work independently while effectively collaborating with physicians, facility staff, and other healthcare professionals.
- Ability to prioritize responsibilities and manage a patient caseload across multiple facilities.
- Commitment to providing compassionate, ethical, and high-quality patient care.
- Prior experience using an electronic medical records (EMR) system.
- Bilingual English/Spanish skills are a plus but not required.
- Ability and willingness to travel between assigned post-acute care facilities.
Reports to the Supervisory Psychiatric Physician / Executive Director.
PHYSICAL REQUIREMENTS- Ability to communicate effectively with patients, families, facility staff, and members of the healthcare team.
- Ability to sit, stand, walk, and move throughout healthcare facilities as required to provide patient care.
- Ability to use a computer, printer, scanner, and other standard office equipment.
- Ability to sit and/or stand and view a computer screen for extended periods.
- Ability to perform repetitive hand and wrist movements for documentation and computer use.
- Ability to use hands and fingers for handling, grasping, and operating office equipment.
- Ability to reach with hands and arms as necessary to perform job duties.
- Ability to travel to and work on-site at multiple post-acute care facilities.
