Job Summary
Under the direction of the Population Health Manager, the Community Health Worker serves as Camino Health Center’s co-located Medi-Cal enroller at Providence Mission Hospital. This role exists to carry out the objectives of the Medi-Cal Community Enrollers grant program, a collaborative partnership between Providence Mission Hospital’s Family Resource Centers (FRCs), the Community Health Initiative of Orange County (CHIOC), and Camino Health Center. The primary focus of this position is expanding Medi-Cal outreach, education, enrollment, and renewal assistance across South Orange County, with particular emphasis on identifying and enrolling patients at Providence Mission Hospital’s Emergency Department who are discharged without coverage. A limited, secondary component of this role supports the Enhanced Care Management (ECM) program in a collaborative capacity.
Essential FunctionsMedi-Cal Enrollment and Outreach (Providence Mission Hospital ED)
- Serves as the co-located Medi-Cal enroller within the Providence Mission Hospital Emergency Department, using live census data access to identify Medi-Cal eligible patients in real time.
- Provides bedside enrollment, renewal support, and benefits education to ED patients prior to discharge to prevent avoidable coverage lapses, addressing a gap in which hundreds of patients are discharged uninsured each month.
- Completes a minimum of 25 (target 30) Medi-Cal enrollment/renewal applications per month; reviews monthly totals with the Population Health Manager for consistency with grant targets and submits totals to CalOptima.
- Documents all patient encounters, in-person interactions, and education contacts in the tracking system developed with the Population Health Manager, including date, location, and encounter type.
- Delivers face-to-face Medi-Cal education and outreach as part of a program-wide goal of reaching 1,000 unduplicated individuals over the grant period; unduplicated counts are reviewed quarterly.
- Is physically present in the community at least two times per month at existing community gatherings and events (outside of sites where enrollment activities take place), coordinating with the Providence Mission Hospital FRC Community Health Worker on consistent messaging.
- Updates and follows a quarterly calendar of Medi-Cal outreach and education opportunities in coordination with Camino and FRC outreach staff.
- Participates in workflow development meetings with the Population Health Manager and Providence Mission Hospital ED leadership to adapt, pilot, and refine the CHW’s integration into the ED, including securing access to required hospital technology and dashboards.
- Completes new-hire onboarding and orientation at both Camino Health Center and Providence Mission Hospital, with a confirmed start date.
Secondary Responsibility: Enhanced Care Management (ECM) Support
- On a limited, as-needed basis, identifies potential ECM candidates and supports eligible ECM members with system navigation and connection to socioeconomic resources.
- Documents member barriers and shares updates with the ECM team, and participates in ECM case reviews when requested by the Population Health Manager.
- Ensures all ECM-related documentation is accurate and compliant with regulatory and accreditation standards.
Minimum Position Qualifications
Education: High school diploma or GED required.
Language: Bilingual (English/Spanish).
Experience/Training: One year minimum experience working in outreach and customer service within the medical office and behavioral health environments with individuals who struggle with chronic illness, substance use disorders and mental health issues is required.
License/Certification: Valid CA Driver License and proof of auto insurance. BLS certification is required.
Preferred Position Qualifications
Experience/Training: Community Health Worker (CHW) certificate. Two years of work experience in a similar role preferred.
Additional Requirements- Able to work flexible hours, including evenings and weekends.
- Able to work at various health center locations and outreach sites.
- Must have reliable personal transportation for home visits and community-based work, as well as travel to and from Providence Mission Hospital.
- Ability to work on-site at Providence Mission Hospital and be granted appropriate hospital system and dashboard access.
- Comfort working in a hospital Emergency Department environment alongside clinical staff.
