  • Multidisciplinary Liaison & Triage: Operating as a primary liaison with the Emergency Department (ED) and inpatient medical floors to streamline referral services, screening incoming EMR patient alerts, and monitoring integrated discharge dispositions.
  • Staff Supervision & On-Site Support: Providing direct operational leadership, hiring, onboarding, training, and performance evaluations for the multidisciplinary team while resolving immediate client or staff milieu concerns.
  • Community & External Partnerships: Building and sustaining highly visible collaborative relationships with community-based organizations, social service providers, local law enforcement agencies, and public stakeholders to maximize program outreach.
  • Regulatory Compliance & Tracking: Securing optimal programmatic functioning to maintain absolute compliance with strict external governing bodies, including state departments, RWJBarnabas Health policies, and The Joint Commission (JCAHO).
  • Data-Driven Performance Reporting: Analyzing and interpreting crucial departmental data metrics to generate regular progress reports highlighting program achievements, funding benchmarks, and growth areas for hospital administration and funders.

  • You possess an advanced executive acumen, excel under pressure, and maintain a highly professional demeanor when handling tight reporting deadlines or cross-functional hospital crises.
  • You set an exceptionally high bar for clinical documentation, quality assurance tracking, and strict regulatory adherence within a grant-funded infrastructure.

  • Experience: Minimum of two (2) years of applicable, post-graduate clinical program management and development experience is required. Previous supervisory, team leadership, or manager-level experience is strictly required.
