Ocala Recruitment is recruiting for a care coordinator on behalf of our client, a well-established not-for-profit residential respite service for adults with learning disabilities, autism and complex needs in Sudbury. Our client is an inclusive service which provides day opportunities and short breaks and is committed to supporting service users to live fulfilled, happy lives.

The Role
The Care Coordinator will assist with all day-to-day operations of our client’s inclusive respite service, supporting with the continued development of their unique short-break model.
You will take the lead on aspects of person-centred care, ensuring the highest possible standards can be achieved in line with best practices, legislature and individual needs. You will play a key role in planning, coordinating, and monitoring care to ensure each young person receives personalised support that promotes independence, choice, dignity, and wellbeing.
As part of this role you will communicate and work closely with multidisciplinary teams, families and care professionals. You will act as manager in the absence of the Registered Manager.
Location and hours
This is an in-person, full-time, permanent role located at the client’s service in Sudbury. 40 hours per week to be worked flexibly.
Job Purpose
The purpose of this role is to coordinate and manage teams to provide the best person-centred care, that promotes the independence of individuals and helps those support users to live a fulfilling life.
Core Responsibilities
Coordinate and oversee personalised care plans, ensuring they are accurate, up to date, and reflect each individual's needs and goals.
Work collaboratively with families, social workers, healthcare professionals, and multidisciplinary teams.
Support the planning and delivery of safe, effective respite stays.
Complete assessments, risk assessments, reviews, and care documentation to a high standard.
Ensure compliance with relevant legislation, safeguarding procedures, and regulatory standards.
Monitor care quality and identify opportunities for continuous improvement.
Support and mentor care staff, promoting best practice and person-centred approaches.
Participate in admissions, transitions, and discharge planning.
Assist with audits, compliance, quality assurance, and service development.
Maintain accurate electronic records and contribute to reporting requirements.
Review relevant documentation for short breaks, ensuring all necessary information is readily available during the service user’s stay
Train staff on the job to meet care and support needs of the service users
Support to cover shifts when required to ensure a safe service
Take active part in the on call to ensure reliable support to the service and staff
Prepare reports and attend review meetings as appropriate, required and delegated

Success in the role is ensuring the smooth running of care and support delivery, ensuring the best standards are achieved. Assisting with the effective organisation and coordination of support for service users and supporting the care team.
Essential requirements
A highly personable individual, skilled with written, telephone and verbal communication
Proactive, self-motivated and enthusiastic individual
Experience working with individuals with autism, learning disabilities, and complex health needs.
Ability to work well under pressure, assess different situations and make decisions appropriately considering the interests of those involved
A NVQ Level 3 in Health and Social Care (or equivalent) and willingness to work towards Level 5 Leadership and Management if not already achieved.
Knowledge of relevant legislation and care standards.
Enhanced DBS check/ willingness to complete prior to commencement of work
Right to work in the UK
Full and clean driving licence and access to a vehicle
Desirable
Previous experience in a care coordination, senior support worker, or supervisory role within health or social care.
Excellent organisational and communication skills.
A sound understanding of person-centred care, safeguarding, positive behaviour support, and risk management.
The ability to build positive relationships with families, professionals, and colleagues.
Confidence using electronic care management systems and maintaining accurate records.
A flexible, positive, and solution-focused approach.
