Description:

The Supportive Housing Services (SHS) Case Manager provides supportive services at a Permanent Supportive Housing complex, The Nel. The SHS case manager will work in collaboration with community providers and housing providers for people with disabilities to obtain and maintain permanent supportive housing.

The schedule for this position is Sunday - Wednesday - 8am – 6:30pm (minus 1/2-hour lunch)

ESSENTIAL JOB FUNCTIONS:

  • Assist participants with all aspects required to stabilize housing with a proactive and assertive engagement approach that will foster successful independent living
  • Fulfill role as liaison between tenants, property management, and treatment teams, advocating for participants with property management, attending to housing issues, and coordinating tenant move-in and move-out with property management
  • Provide individual case management to participants, including developing/modifying transitional plans, housing stability plans, and employment plans
  • Continually monitor and evaluate each participant’s progression through their individualized housing stability plan and communicate with program coordinator about the needs, experiences, and observations of program participants
  • Provide skills training and coaching for medication management, financial management, and all activities of daily living, with the goal of maintaining permanent housing
  • Provide crisis management and interventions as needed and respond immediately when participant shows sign of decompensation or is at imminent risk of eviction
  • Complete required documentation including strengths assessment, individualized housing stability plan, safety plan, and daily progress notes, within established timelines

OTHER JOB FUNCTIONS:

  • Provide appropriate referrals to community agencies
  • Provide transportation to individuals as needed
  • Identify and obtain community resources
  • Collaborate with staff members, community agencies/supports, and family members about participants mutually served
  • Participate in agency and staff team meetings
  • Other duties as assigned

KNOWLEDGE AND ABILITIES:

  • Able to communicate appropriately with staff and clients, including individuals with severe mental health symptoms and substance use issues
  • Experience working with multiple agencies and various populations
  • Able to provide supportive, professional, and appropriate services to individuals
  • Able to offer understanding and respond effectively and with sensitivity to special populations, such as those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, disability, migrant and seasonal worker status, homelessness, people living with HIV/AIDS, and others
  • Able to effectively engage clients by telephone or in person
  • Able to manage workload, including scheduled commitments
  • Able to use mobile technology, computers, and standard office equipment
  • Able to enter documentation into an electronic records system.
  • Able to multitask and complete a variety of tasks each day (e.g. phone calls, outreach, referrals, documentation, etc.)
  • Able to work independently as well as a member of a team
  • Able to be a resource for others and work with providers in the community, as well as knowledge of community resources
  • Able to work within HIPAA and confidentiality requirements as they relate to supportive housing
  • Able to follow policies and procedures
  • Able to meet Universal Expectations of Employment
  • Able to perform physical requirements of the position, including lifting up to 25 pounds, standing, sitting, bending, kneeling, crouching, reaching, grasping, using fingers, tolerating strong odors, speaking, repetitive motion, extensive use of computer, local travel
Requirements:

Minimum Requirements:

Education and experience – must have at least one of the following:

  • Lived experience of homelessness, mental health, and/or substance use plus a combination of 2 years of related employment, education, or volunteer work
  • BA/BS degree in mental health, social services, or related field, or equivalent, plus one year of experience relating to or assisting individuals with mental illness, housing instability, and/or substance use concerns; OR
  • Eligible to be certified as a Qualified Mental Health Associate through Mental Health and Addiction Certification Board of Oregon or meet the criteria to be eligible for certification after one year; OR
  • Valid Oregon Driver License in accordance with Laurel Hill Center vehicle use policy and procedures
  • Documentation of successful completion of a background and fingerprint check as specified by Oregon law, as well as a urinalysis drug screen (Although recreational marijuana is legal in Oregon, our policy—aligned with both federal and state regulations—requires all applicants to test negative for THC prior to employment)
