This is a unique opportunity to play a pivotal role in shaping the operations of a state-of-the-art facility that brings together Psychology, Physical Medicine and Rehabilitation (PM&R), Neurology, and Psychiatry under one roof. We need a strong leader who is ready to make an impact, set high standards, and foster a culture of excellence in patient care.
We are seeking a dedicated and qualified Case Manager to join our team!

PRIMARY RESPONSIBILITIES
- Interfacing with referring medical clinics, physicians, insurance adjusters and patients.
- As part of a multidisciplinary team, supports our physicians in the delivery of care to patients.
- Obtaining authorizations for treatment.
- Obtaining medical records.
- Verifies patient information, history taking, confirmation of current condition.
- Coordinate patient care with scheduler.
- Arranging patient transportation
- Counsels patients on physician orders.
- Updates and verifies patient information in our EMR system
- Following up on medical appointments, authorizations and requests for medical records
- Providing support to coworkers.
- To be cross-trained and back up to co-workers
REQUIRED QUALIFICATIONS
- High school graduate.
- Strong writing skills.
- Bilingual in English and Spanish.
PREFERRED QUALIFICATIONS
- Workers' compensation: 1 year (Preferred)
- Strong written and verbal communication skills
- Team player
- Personable and professional
- Consistently positive attitude
- Enjoys working in a dynamic team environment
- Organized and efficient.
