Responsibilities
- Perform comprehensive occupational therapy evaluations in the home setting
- Develop individualized treatment plans based on patient goals and clinical needs
- Provide skilled occupational therapy services in patients' homes
- Improve patients' ability to safely perform Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
- Recommend adaptive equipment and home modifications to enhance safety and independence
- Educate patients and caregivers on therapeutic techniques, fall prevention, energy conservation, and home safety
- Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and other healthcare professionals
- Monitor patient progress and modify treatment plans as needed
- Maintain accurate and timely clinical documentation
- Complete patient notes and communications within required timeframes
- Ensure compliance with agency policies and applicable regulations
- Travel to patient homes within assigned coverage areas

Qualifications
- Active Pennsylvania Occupational Therapist (OT) License
- Graduate of an accredited Occupational Therapy program
- Home Health experience preferred but not required
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal skills
- Strong computer and documentation skills
- Reliable transportation and valid driver's license
- Ability to work independently and effectively manage a flexible schedule
- Commitment to delivering exceptional patient-centered care
