San Diego - Home Care Liaison (HCL)
Coast Care Home Care Liaison (HCL) Full-Time, Exempt Starting Salary:$75,000 annually Benefits: Eligible for Coast Care's full-time employee benefits package San Diego County, California; community-based position with regular local travel Reports To: COO Position Summary The Home Care Liaison (HCL) serves as a key representative of Coast Care Partners in the community and is responsible for developing and maintaining referral relationships that support sustainable growth across the organization's home care programs. The HCL builds trusted partnerships with hospitals, skilled nursing facilities, rehabilitation centers, physician offices, health plans, case managers, social workers, senior living communities, community organizations, and other referral sources. This position combines business development, community outreach, relationship management, and service coordination. The HCL educates referral partners and families about Coast Care's services, identifies opportunities to connect individuals with appropriate care, supports a responsive referral experience, and works closely with internal teams to promote timely service initiation. The successful candidate is professional, compassionate, organized, results-oriented, and aligned with Coast Care's core values of Reliability, Empathy, Improvement, and Respect. Essential Duties and Responsibilities Referral Development and Business Growth - Develop and execute a strategic outreach plan to increase qualified referrals and admissions. - Identify, cultivate, and maintain productive relationships with new and existing referral partners. - Conduct regular in-person visits, presentations, educational meetings, networking activities, and follow-up communications throughout the assigned territory. - Clearly communicate Coast Care's services, programs, eligibility requirements, service areas, and value proposition. - Identify new market opportunities and recommend outreach strategies that support organizational growth. - Meet or exceed established goals for referral activity, qualified referrals, admissions, relationship development, and census growth. Referral and Service Coordination - Serve as a responsive point of contact for referral partners, prospective clients, families, and community stakeholders. - Gather accurate referral information and promptly communicate it to the appropriate internal department. - Coordinate with Contracts, Recruitment, Client Services, Scheduling, and other teams to support timely follow-up and service initiation. - Monitor referral progress, identify barriers, and help facilitate appropriate resolution or escalation. - Maintain professional communication with referral sources throughout the intake and onboarding process. - Protect confidential client and business information and follow all applicable privacy, HIPAA, and company requirements. Community Engagement and Brand Representation - Represent Coast Care at community events, professional associations, healthcare meetings, resource fairs, and networking functions. - Build awareness of Coast Care's mission and services within the communities served. - Develop strong knowledge of local healthcare resources, referral networks, competitors, and community needs. - Present Coast Care professionally and consistently in all interactions and marketing activities. - Partner with leadership and marketing staff on outreach materials, campaigns, events, and community initiatives. Tracking, Reporting, and Accountability - Maintain complete and accurate records of outreach activities, contacts, referrals, follow-ups, and outcomes in designated systems. - Provide regular reports on pipeline activity, referral trends, conversion results, relationship development, and market opportunities. - Use data to evaluate outreach effectiveness and adjust strategies as needed. - Participate in leadership meetings, growth planning, and cross-department coordination. - Maintain an organized schedule and consistently follow through on commitments. Qualifications Required - Bachelor's degree in healthcare administration, business, marketing, social services, communications, or a related field; relevant professional experience may be considered in place of a degree. - At least three years of experience in home care, healthcare business development, community outreach, provider relations, case management, senior services, or a related field. - Demonstrated ability to build professional relationships and generate referrals or business growth. - Strong verbal, written, presentation, follow-up, and interpersonal skills. - Ability to work independently in the community while collaborating effectively with internal teams. - Strong organizational skills and ability to manage multiple priorities in a fast-paced environment. - Proficiency with Microsoft Office, Google Workspace, CRM systems, and electronic documentation tools. - Valid driver's license, reliable transportation, and ability to travel throughout the assigned territory. - Ability to attend occasional early-morning, evening, or weekend community events as business needs require. Preferred - Existing professional relationships within the San Diego County healthcare, senior care, disability services, or community-based care network. - Experience working with private-duty home care, Medi-Cal, CalAIM, HCBS, regional centers, health plans, or other community-based service programs. - Knowledge of home care referral, intake, authorization, onboarding, and service-start workflows. - Bilingual communication skills.
