This role focuses on advancing evidence-based, cost-effective medication management for patients with kidney and cardiac conditions and related comorbidities. You will work at the intersection of clinical pharmacy, population health, and care management to optimize medication regimens and improve patient outcomes. The position involves addressing medication access, affordability, education, adherence, and safety while supporting transitions of care and chronic disease management. You will collaborate closely with physicians, pharmacists, care teams, and other healthcare professionals to identify opportunities for improved medication use. The role also contributes to pharmacy strategy, clinical programs, treatment guidelines, and quality improvement initiatives. This is a fully remote opportunity for an experienced clinical pharmacist with strong analytical, communication, and strategic skills.

• Review medication regimens and recommend adjustments based on clinical protocols, current guidelines, and evidence-based practices to optimize kidney and cardiac disease management.
• Develop educational resources for providers, members, and staff covering new medications, indications, guideline updates, and relevant clinical information.
• Contribute to the development of treatment algorithms, clinical guidelines, policies, procedures, and disease management initiatives.
• Respond to pharmacy referrals involving medication access, affordability, education, adherence, optimization, and safety.
• Evaluate high-cost, high-risk, and high-volume medications using a population health approach to identify opportunities for improved outcomes and value.
• Improve medication reconciliation and adherence processes, particularly during transitions of care and ongoing chronic disease management.
• Collaborate with physicians, care managers, and other healthcare professionals to share clinical pharmacy expertise and support coordinated care.
• Oversee pharmacy technicians and provide appropriate direction and support.
• Participate in quality improvement, process enhancement, and other pharmacy or clinical initiatives as assigned.
• Maintain compliance with applicable policies, procedures, state and federal regulations, and requirements governing the confidentiality and protection of patient health information.
• Complete required compliance and other organizational training and perform additional responsibilities as needed.

Requirements:

• Active pharmacist license.
• Doctor of Pharmacy (PharmD) degree from an accredited School of Pharmacy.
• At least 5 years of clinical pharmacy experience involving direct collaboration with physicians and other healthcare providers in hospital or ambulatory care settings.
• Experience in renal patient pharmacy management and value-based care is preferred.
• Experience with health plans and/or healthcare vendors is preferred.
• ASHP-accredited pharmacy residency experience is preferred.
• Understanding of CMS Star measures, HEDIS measures, MIPS, and National Quality Forum measures.
• Excellent written and verbal communication skills, with the ability to communicate clinical information clearly to diverse audiences.
• Strong organizational skills, attention to detail, and the ability to manage multiple priorities effectively.
• Strong analytical, strategic, and problem-solving capabilities with a metrics-driven approach.
• Ability to work independently while collaborating effectively across multidisciplinary teams.
• Proficiency with Microsoft Office applications, including Outlook, Word, Excel, PowerPoint, and Teams.
• Ability to work remotely and perform computer-based tasks requiring extended periods of sitting and repetitive typing or texting.
