Job Description

PRIMARY PURPOSE

The Recovery Care Coordinator at Lorain County Health & Dentistry (LCH&D) plays a central role in supporting patients engaged in Medication-Assisted Treatment (MAT) services. This position is responsible for meeting with patients during MAT orientation and providing ongoing support in coordination with Primary Care Providers (PCPs) during follow-up visits. The Recovery Care Coordinator delivers trauma-informed, culturally responsive care coordination to promote patient engagement, adherence to treatment, and positive health outcomes.

This role also supports the growth and development of the MAT program through community outreach, marketing efforts, and partnership-building. This position is grant-funded and focused on increasing access to care, improving retention, and expanding awareness of MAT services within the community.

Essential Functions:

Patient Care Coordination

  • Conduct MAT program orientations, reviewing program expectations, treatment structure, and available supports
  • Meet with patients during PCP visits to reinforce treatment adherence and address barriers to care
  • Collaborate with behavioral health providers, PCPs, and additional care teams to support integrated, patient-centered care
  • Provide trauma-informed, culturally sensitive support to patients with substance use disorders
  • Assist with coordination of internal and external referrals, including behavioral health and community resources
  • Conduct outreach to patients who miss appointments to support re-engagement in care

Medication Coordination and Prior Authorization

  • Coordinate with onsite and external pharmacies to support timely access to MAT medications
  • Schedule and track deliveries of injectable medications to ensure availability for patient appointments
  • Verify that prescriptions are sent accurately and timely, including correct medication, dosage, and pharmacy location
  • Complete prior authorizations for MAT medications and follow up to ensure timely approval
  • Assist patients with enrollment in copay assistance programs and other medication access resources as needed
  • Collaborate with nursing staff to track medication deliveries and confirm medications arrive at the correct location prior to scheduled appointments

Program Support and Development

  • Support implementation and ongoing improvement of MAT program workflows and processes
  • Assist in identifying opportunities to enhance patient engagement and program effectiveness
  • Participate in quality improvement initiatives and team meetings

Community Outreach and Engagement

  • Support outreach efforts to increase awareness of MAT services within the community
  • Assist with development and distribution of program materials
  • Build and maintain relationships with community partners and referral sources
  • Represent LCH&D at community events and outreach activities as needed

Grant-Funded Program Responsibilities

  • Support data collection and documentation to meet grant requirements
  • Assist in tracking program outcomes, including patient engagement and retention
  • Ensure compliance with grant-related expectations and reporting activities

Qualifications:

  • Education: Bachelor’s degree in social work, behavioral health, public health, or related field required
  • Certificates, licenses, and registration: Relevant certification or licensure (e.g., CDCA, LCDC, LSW, LPC) preferred
  • Valid Ohio driver’s license preferred
  • Experience working with substance use disorders, behavioral health, or integrated care settings preferred

Skills and Abilities

  • Strong interpersonal and communication skills
  • Knowledge of substance use disorders and recovery-oriented care
  • Ability to work effectively within a multidisciplinary team
  • Strong organizational skills and attention to detail
  • Ability to manage multiple tasks in a fast-paced environment
  • Relevant certification or licensure (e.g., CDCA, LCDC, LSW, LPC) preferred
  • Valid Ohio driver’s license preferred

This position is grant-funded and contingent upon continued funding. The role supports the expansion and sustainability of MAT services at LCH&D and may evolve based on program needs.
