JASA is one of NYC’s largest and most trusted nonprofits serving older adults. Our dedicated professionals help improve the lives of thousands of seniors through impactful, mission-driven work.

  • Meaningful Frontline Impact: Provide essential advocacy, care coordination, and protective oversight that enable older adults to age safely, comfortably, and with dignity in their own homes and communities.

  • Flexible Community-Based Practice: Enjoy a balanced part-time schedule (2 days per week) while delivering critical in-home and community-based case management across NYC neighborhoods.

  • Professional Growth & Collaboration: Work alongside skilled social workers, multidisciplinary partners, and community providers while gaining experience with standardized municipal assessment tools (EISEP-PATH, PSA).

📝 What You’ll Be Doing

JASA is seeking a compassionate, detail-oriented Case Manager to join our community services team on a part-time basis (two days per week). Operating in accordance with the standards of the social work profession and specific government funding requirements, you will conduct in-home and office-based psychosocial assessments, build comprehensive written service plans, and connect older adults to vital community resources.

This role serves as a crucial advocate and guide for older adults and their families, addressing challenges related to chronic impairment, social isolation, and financial strain while navigating complex public benefit systems.

Key Responsibilities:

  • Standardized Client Assessments: Conduct thorough in-home, in-office, and community assessments of the social and emotional needs of older adults and their families utilizing standardized instruments (such as PSA and EISEP-PATH).

  • Comprehensive Case Planning & Counseling: Develop, implement, and maintain individualized service plans providing supportive guidance, coping tools, and problem-solving strategies that preserve autonomy and daily functioning.

  • Benefits & Entitlements Advocacy: Screen older adults for benefit eligibility; assist with applications, recertifications, and advocacy for Medicaid, Medicare, SNAP, Cash Assistance, Home Care, Meals on Wheels, SCRIE/DRIE, and emergency cash relief.

  • Direct Service Arrangement: Coordinate direct supportive services, including homecare attendants, medical transportation, housing assistance, legal aid referrals, and psychiatric consultations.

  • Capacity Evaluation & APS Intervention: Evaluate clients’ cognitive and physical capacity to manage their daily affairs and protect themselves from exploitation or neglect; initiate rapid escalations and referrals to Adult Protective Services (APS) when indicated.

  • Ongoing Reassessment & Monitoring: Conduct scheduled reassessments, evaluate service efficacy, adjust care goals as client needs evolve, and apprise clinical supervisors immediately of emergent crisis situations.

  • Group Facilitation & Community Outreach: Conduct community education workshops and group counseling sessions; represent JASA at community forums, inter-agency meetings, and unit conferences.

  • Electronic Documentation: Maintain complete, audit-ready client records, case notes, progress reports, and statistical deliverables within web-based case management databases in compliance with JASA and funder timelines.

🎓 What You’ll Need

  • Education: Bachelor’s Degree in Social Work (BSW), Human Services, Psychology, Gerontology, or an equivalent Bachelor’s-level degree from an accredited institution required.

  • Experience: Prior experience in case management, social services, or community-based eldercare preferred.

  • Field Mobility: Ability to travel throughout New York City via public transportation to conduct client home visits and independently negotiate stairs and environmental barriers in residential buildings.

  • Technical Literacy: Computer proficiency in Microsoft Office Suite (Word, Excel, Outlook) and the ability to learn and use specialized, program-specific client databases.

  • Communication & Cultural Competence: Strong written and verbal communication skills; solid case documentation skills; bilingual proficiency in Spanish is a strong plus.

  • Values & Sensitivity: A deep understanding and appreciation of how race, religion, ethnicity, gender, sexual orientation, and personal values influence client care and family dynamics.

JASA offers competitive compensation and supportive benefits for part-time team members:

  • Wellness Support: Access to Employee Assistance Programs (EAP) and organizational wellness initiatives.

  • Retirement Planning: Eligibility to contribute to a 403(b) retirement plan with a Roth option.

  • Paid Leave: Prorated statutory paid leave and designated agency holidays.

  • Career Advancement: Opportunities for clinical supervision, training workshops, and internal career development.
