Job Overview:The Patient Navigator will identify, coordinate and link Hepatitis B, C and HIV positive clients to medical care from all access points, primarily the Emergency Department, PN will identify positive patients, locate and establish communication with them and provide individualized support for clients to overcome barriers to initial care, deliver health education to increase linkage success and will serve as the primary contact and liaison for clients and the care team to ensure prompt and successful connection to first medical appointment,
Qualifications:
Required:
- Bachelor’s degree preferred or equivalent, training in a related social service or human service field
- Experience working in a healthcare with the community
Scheduling Requirements:
- Day shift with the need to occasional flexibility to accommodate community events, patient schedule preference
Essential Functions:
- Identifying and engaging appropriate patients
- Linking& Re-Linking new and previously diagnosed patients to medical care
- Providing patient education regarding diagnosis and treatment
- Facilitating care coordination including arranging transportation, providing support for insurance, accompanying patients to appointments and providing reminder phone calls, texts, emails
- Initiate appropriate supportive referrals such as substance abuse treatment, mental health, PrEP
- Serving as primary liaison between the screening, diagnostic and treatment teams throughout the continuum of testing to initial treatment
- Documenting services delivered to patients in the electronic health record and maintaining accurate data for required reporting, establishing best practice and dissemination of project outcomes
- Contributing to institutional policy change to support routine screening and linkage to care
- Candidate must have the ability to demonstrate the delivery of culturally sensitive services, be able to function effectively within a team-based, inter-professional approach and possess a strong knowledge of local resources to facilitate client referrals to
