Position Title
Licensed Practical Nurse (LPN)

Position Summary
The Licensed Practical Nurse (LPN) is responsible for providing nursing support services to residents in accordance with applicable state regulations, physician orders, community policies, and individualized service plans. The LPN helps promote residents’ health, safety, dignity, and independence while supervising care staff and ensuring quality clinical services. This role includes medication administration/passing meds, monitoring resident conditions, and communicating with families and healthcare providers as needed.
Essential Duties and Responsibilities
Clinical Care
Administer medications, treatments, and injections as ordered by physicians and permitted by state regulations.
Perform routine medication pass accurately and timely while following the “Five Rights” of medication administration.
Monitor residents for side effects, adverse reactions, and changes in condition.
Document medication administration and clinical observations in the electronic health record/MAR system.
Assist with resident assessments, re-assessments, and care plan updates.
Respond to resident emergencies and provide first aid within scope of practice.
Coordinate physician orders, pharmacy needs, labs, and follow-up appointments.
Resident Support
Promote resident comfort, dignity, privacy, and independence.
Communicate respectfully with residents, families, and responsible parties regarding health concerns.
Support residents with wellness monitoring such as vitals, weights, skin checks, and general observation.
Supervision & Leadership
Supervise and delegate tasks to CNA/HHA/caregiver staff in accordance with regulations and policy.
Ensure resident care staff follow service plans and safety procedures.
Report incidents, concerns, and changes in resident status promptly to management.
Participate in shift report and interdisciplinary meetings.
Compliance & Documentation
Maintain accurate nursing notes, medication records, incident reports, and regulatory documentation.
Ensure compliance with state assisted living regulations, infection control practices, and OSHA standards.
Safeguard controlled substances and report discrepancies immediately.
Participate in audits, in-services, and quality improvement initiatives.
Qualifications
Current active LPN license in the State of New Jersey (or applicable state).
CPR certification required.
Experience in assisted living, long-term care, memory care, or geriatrics preferred.
Knowledge of medication administration systems and MAR/eMAR documentation.
Strong communication, leadership, and customer service skills.
Physical Requirements
Ability to stand, walk, bend, lift, and assist residents as needed.
Ability to respond quickly in emergency situations.
Ability to push medication cart and complete medication pass rounds.
Work Environment
Assisted living community, including resident apartments, common areas, and memory care neighborhoods.
May require weekends, holidays, and flexible shifts.
