Provides medication therapy management services in collaboration with providers to assure that drug therapy is safe, effective, efficient and cost-effective. Pharmacy services provided include medication therapy management visits with patients, medication reconciliation, medication therapy monitoring, medication education, medication access, and various scholarly activities. The Pharmacist performs both clinical and distributive tasks necessary to ensure that patients’ medication therapy is safe, effective, timely and efficient. The Pharmacist acts independently within the scope of their license and areas of expertise with the help of general policies, procedures and practice guidelines with support from the Pharmacy Director.
