Remote Opportunity! Monday - Friday, 8am - 5pm

PRIMARY FUNCTION

The Care Manager, Behavioral Health is responsible for providing care management and care coordination services to patients with serious mental illness and/or substance abuse who have high rates of behavioral health utilization and/or psychosocial vulnerability. The Care Manager focuses on decreasing hospital admissions, increasing medication compliance, and improving quality of life and care coordination.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Conducts comprehensive patient assessments. Reviews clinical appropriateness and urgency of referrals. Performs warm calls/wellness checks in between provider visits with patients to escalate any new clinical concerns to providers, monitor treatment compliance and patient status.

  1. Performs clinical intervention by developing a care management treatment plan that is responsive and targeted to the patients’ needs.

  1. Assesses, plans, implements, monitors, and evaluates options and services to meet the patient’s behavioral health needs.

  1. Assists with referrals to external behavioral health/substance use community-based resources to assist with accessing quality and cost-effective care. Ensures we steer our patients to our in-network mental health specialty partners and aids in clinical assessment to match urgency and severity of the case.

  1. Collaborates with the patient and patient care teams, both internal and external, to ensure appropriate actions are taken to meet the patients’ needs. Any concerns are escalated to the internal team. Maintains regular communication with external providers to ensure coordinated care.

  1. Addresses barriers to healthcare including, but not limited to, homelessness, housing instability, financial resources, transportation, childcare, etc. Coordinates with specialty community partner care manager to address barriers to care. Internally, we are also able to refer to 211.

  1. Documents patient’s psycho/social assessment and progress notes including changes in medical psycho/social functioning, progression, patient’s family, and referrals to internal and external agencies.

  1. Serves as a source of behavioral health knowledge for internal staff.

  1. Provides information for Crisis Intervention and Support for children and adolescents.as needed and communicates with clinical team for coordinated care.

QUALIFICATIONS

EDUCATION:

  • Minimum Bachelor’s degree in Psychology, Nursing or related field.

EXPERIENCE:

  • A minimum of 2-3 year of healthcare experience required with experience in a behavioral health setting.

LICENSURE / CERTIFICATION

  • Florida License Requirement: RN or ARNP.

KNOWLEDGE, SKILLS, AND ABILITIES

  • Strong clinical, interpersonal, organizational and communication skills.

  • Strong English writing skills with excellent spelling, grammar, and punctuation.

  • Basic Excel and knowledgeable in Microsoft Outlook, and Word.

  • Willing to learn the quality process as it relates to a medical practice.

  • Strong computer skills and experience with electronic health records.

  • Ability to work independently and as a team player.

  • Detail oriented and ability to multi-task.

  • Possess a positive attitude and display exceptional customer service skills.

  • Strong telephonic skills and verbal articulation skills.

  • Ability to assess emergency situations and act accordingly.

  • Able to build effective working relationships with patients, parents, guardians, fellow employees and the public.

TYPICAL WORKING CONDITIONS

  • Patient-facing

  • May be full time remote/telework.

  • Must be U.S. based.

  • Full time remote requires quiet workspace without distractions or interruptions while interacting with patients and caregivers.

  • Considerable sitting on a daily basis

  • Able to work under emotionally stressed conditions.

  • Occasionally subject to verbal abuse, threats, and physical violence from angry, hostile, or disgruntled patients and/or family members.

OTHER PHYSICAL REQUIREMENTS

  • Vision

  • Sense of sound

  • Sense of touch

  • Ability to wear Personal Protective Equipment (PPE) if applicable.

PERFORMANCE REQUIREMENTS

  • Demonstrates cultural sensitivity and respect for the patient.
