LVN Responsibilities

• Administer medications and treatments as prescribed
• Monitor resident conditions and report changes
• Provide direct patient care and support daily living needs
• Document care and medication administration accurately
• Support and follow individualized care plans
• Supervise and guide CNAs/caregivers as needed
• Ensure compliance with all regulatory and facility standards
• Respond to emergencies and maintain a safe care environment
• Communicate effectively with residents, families, and care teams
• Performs other LVN duties as assigned

Qualifications:

• Active LVN license in the state of Texas
• Minimum of 6 months of experience in a long-term care, assisted living, or skilled nursing setting
• Strong clinical and communication skills
• Ability to work independently and follow care plans
• Must be able to lift 50 lbs and assist residents with mobility
• Familiarity with electronic medical records (EMR) systems is a plus
• CPR certification preferred
