About This Position

CPLC Corazón is hiring Licensed Practical Nurses (LPN)! CPLC Corazón is a client-centered center with bilingual staff where all cultures are welcome and celebrated. We have been providing Substance Abuse treatment since 1983, offering a variety of treatment modalities.

CPLC Corazón features a range of substance abuse services, including group, individual, family, and couples therapy, in addition to nightly community 12-step support groups. Our 65-bed, ADA accessible residential facility provides all meals, an on-site laundry facility free of charge, a basketball court, and a garden. We offer holistic health techniques including yoga, tai chi, chi gong, and sweat lodge.

POSITION SUMMARY
To greet all members attending appointments with their provider, collaborate with nursing staff and assist as needed. Coordinate care with outside providers and assist with administrative duties as they pertain to meeting member needs. Collaborate with the clinical teams and their support staff. Promote health and wellness practices to the members we serve. Focus is on ensuring the members have a pleasant experience while in the clinic.

MINIMUM QUALIFICATIONS

  • Have a current license by the Arizona State Board of Nursing as a LPN
  • Have at least 1 year experience performing medical duties


SCHEDULE AVAILABILITY

  • Wednesday-Saturday afternoons, or
  • Sunday-Wednesday mornings or afternoons


JOB DUTIES

  • Complete all clinical documentation according to company standards for services provided including nursing special notes, nursing contact notes and nursing assessments
  • Assist in client admission coordination and documentation
  • Coordination and integration of care for clients with other community providers
  • Actively participate in program activities as directed to enhance client care
  • Provide integrated health education including correlation between mental disorders and medical disorders, effects and side effects of psychiatric medication, and other medical needs to enhance client health and wellbeing as needed
  • Coordinate client care including medication recommendations and assisting clients in coordinating medical and/or other provider appointments as needed
  • Provide assistance to clients in self-administration of medications and PRN OTC medications for clients as required
  • Maintain LPN licensure, First Aid and CPR Certifications in good standing
  • Respond expeditiously in medical emergencies in compliance with company standards and/or best practices, including Incident Reporting procedures
