About the Role

We are seeking an experienced Clinical and Services Coordinator (NDIS) to join our VIC team. Reporting to the Business Relationship Manager – VIC, this role is ideal for a Registered Nurse who is confident in clinical coordination, service delivery oversight, and multidisciplinary care coordination within a fast-paced disability support environment.

You will work closely with participants, families, support workers, Support Coordinators, allied health professionals, and other stakeholders to ensure safe, consistent, and person-centered service delivery across SIL, SDA, and community-based supports.

This role combines clinical coordination and operational service delivery, with a strong focus on participant wellbeing, risk management, documentation oversight, staff capability development, and service quality improvement in line with NDIS Practice Standards and Accord Care policies.

Key ResponsibilitiesClinical Coordination & Participant Support
  • Coordinate multidisciplinary care with allied health professionals, GPs, families, Support Coordinators, and other stakeholders
  • Support participants with complex and high-intensity needs, monitoring wellbeing, clinical risk, and outcomes
  • Support care planning, risk assessments, and behaviour support implementation aligned with participant goals
  • Promote participant rights, dignity, choice, and person-centered practice
  • Support safe medication administration, personal care, and high-intensity supports
  • Identify, escalate, and manage clinical concerns, incidents, and risks in line with procedures
Service Delivery & Operational Support
  • Support day-to-day service delivery across SIL, SDA, and community-based services
  • Assist with participant intake, onboarding, and transitions to ensure continuity of care
  • Support rostering coordination and workforce coverage
  • Assist in resolving operational issues impacting service delivery
  • Promote consistent service delivery aligned with participant plans and organisational standards
Staff Training, Competency & Clinical Support
  • Provide clinical guidance, coaching, and support to support workers
  • Facilitate training in medication administration, clinical care procedures, and best-practice support delivery
  • Support competency assessments to ensure safe and compliant care
  • Support onboarding and ongoing staff capability development
Compliance, Documentation & Quality Assurance
  • Maintain accurate documentation including progress notes, care plans, incident reports, and risk assessments
  • Ensure compliance with NDIS Practice Standards, organisational policies, and relevant clinical guidelines
  • Participate in audits, incident reviews, and corrective actions
  • Contribute to continuous improvement and quality initiatives
Stakeholder Engagement & Coordination
  • Liaise with participants, families, Support Coordinators, allied health professionals, hospitals, and SIL providers
  • Participate in intake, review, and transition planning meetings
  • Support onboarding and implementation of new services
  • Represent Accord Care professionally within the Victorian disability sector
About You

You are a compassionate and confident Registered Nurse who thrives in a coordinated and fast-paced environment. You are skilled in balancing clinical oversight with service delivery coordination and are committed to improving outcomes for people living with disability.

Essential Requirements
  • Bachelor of Nursing or equivalent qualification
  • Current registration with AHPRA as a Registered Nurse
  • Minimum 2–3 years’ experience in healthcare, disability, aged care, or community settings
  • Experience working with complex care and high-intensity supports
  • Strong understanding of clinical risk, care planning, and medication management
  • Excellent documentation and communication skills
  • Ability to manage competing priorities in a fast-paced environment
  • Current NDIS Worker Screening Check
  • Current Working with Children Check
  • Current driver’s licence
Highly Desirable
  • Experience in SIL, SDA, or NDIS disability services
  • Experience in staff training, mentoring, or competency assessment
  • Understanding of NDIS Practice Standards and Quality Indicators
  • Experience in clinical auditing or quality improvement
  • Postgraduate qualifications in nursing, community health, or disability (advantageous)
