Job Description

The Recovery Specialist serves as a mentor, advocate, educator, and coordinator for individuals affected by substance use disorders. This position provides recovery support, patient engagement, care coordination, education, community outreach, and grant-related administrative support to promote long-term recovery and positive health outcomes. The Recovery Specialist collaborates with healthcare providers, community partners, and internal team members to ensure continuity of care and compliance with program and grant requirements.

Responsibilities:

  • Serve as a role model, mentor, advocate, and motivator for individuals in recovery to support long-term recovery and relapse prevention.
  • Assistindividuals inidentifyingrecovery goals, strengths, barriers, and available resources.
  • Coordinate care and support services before, during, and after patient encounters.
  • Connect individuals to healthcare, behavioral health, medication-assisted treatment (MAT), prevention services, and community resources.
  • Provide support during patient encounters, including admissions paperwork, assessments, vitalsignscollection (as trained and authorized), patient observation, and follow-up activities.
  • Deliver educationregardingsubstance use disorders, opioid use disorders, recovery resources, and wellness strategiestopatients, families, and community members.
  • Collaborate with hospitals, emergency departments, schools, community organizations, and other referral partners to coordinate services and improve access to care.
  • Participate in community outreach activities, educational presentations, and awareness initiatives.
  • Collect, track,maintain, aggregate, and report program data, documentation, and grant deliverables.
  • Maintainaccuraterecords, reports, and supporting documentationin a timely manner.
  • Assistwith grant-funded projects, special initiatives, and community-based programs.
  • Coordinate meetings, referrals, and communication among patients, providers, and community partners as needed.
  • Maintain strict confidentiality andcomply withall applicable privacy and ethical standards.
  • Demonstrate professional boundaries and ethical conduct in all interactions.
  • Participate in required safety programs and emergency response activities as assigned.

Required Skills/Abilities:

  • Ability to motivate, mentor, and advocate for individuals in recovery.
  • Strong interpersonal, communication, and customer service skills.
  • Ability toestablisheffective working relationships with patients, families, healthcare providers, and community partners.
  • Knowledge of recovery-oriented systems of care and community resources.
  • Ability tomaintainaccuratedocumentation and reports.
  • Strong organizational, multitasking, and time-management skills.
  • Proficiencyin Microsoft Office applications, including Word, Excel, and Outlook.
  • Ability to work independently and collaboratively within a team environment.
  • Ability tomaintainconfidentiality and exercise sound professional judgment.
  • Strong attention to detail and problem-solving skills.
  • Regular and predictable attendance.

Education/Experience:

  • High school diploma or GEDrequired.
  • Certification or training in peer recovery support, behavioral health, social services, or a related field preferred.
  • Experience in peer support, recovery coaching, case management, healthcare coordination, behavioral health, or community outreach preferred.
  • Experience with grant documentation, reporting, and data collection preferred.

Physical Demands:

  • Prolonged periods of sitting, standing, walking, and working at a computer.
  • Frequent use of hands and fingers for typing, documentation, and handling materials.
  • Ability to communicate effectively in person, by telephone, and through electronic communication.
  • Occasional lifting andcarrying ofup to 25 pounds.
  • Occasional travel within the service area for community outreach, meetings, training, and grant-related activities.
