Global Meals – Indiana

Position Summary

The Manager, Community & Case Management Partnerships serves as the primary relationship manager for Global Meals throughout Indiana, responsible for developing and strengthening partnerships with statewide case management organizations (CMOs), Medicaid funding sources, Area Agencies on Aging (AAAs), senior residential communities, and community-based organizations.

This field-based position is responsible for driving referral growth, increasing meal utilization, and expanding market penetration through education, outreach, and strategic relationship management.

Support Global Meals' strategic expansion within Indiana by increasing community engagement, strengthening referral partnerships, improving utilization, and driving sustainable growth through local market activation.

Business Impact

This position is expected to drive measurable growth through relationship development with statewide case management organizations, Medicaid funding sources, and community partners, resulting in increased referrals, authorizations, utilization, and market penetration.

Growth & Relationship Management

  • Serve as the primary relationship manager for Indiana's statewide case management organizations.
  • Build and maintain strategic relationships with Medicaid funding sources, Area Agencies on Aging (AAAs), community organizations, and senior residential communities.
  • Identify opportunities to increase meal utilization and expand referral pipelines.
  • Support retention and expansion of existing partnerships through ongoing engagement and education.

Customer Engagement & Field Activity

  • Conduct face-to-face in-services and educational presentations.
  • Participate in community outreach events and networking opportunities.
  • Meet regularly with referral partners to reinforce relationships and increase awareness of Global Meals services.
  • Present program offerings and eligibility requirements to stakeholders.

Referral & Pipeline Management

  • Develop and manage referral pipelines throughout the assigned territory.
  • Monitor referral activity and identify opportunities for increased utilization.
  • Collaborate with internal teams to ensure timely onboarding and service delivery.
  • Track performance against established KPIs.

Marketing & Community Presence

  • Represent Global Meals at community events, conferences, and health fairs.
  • Support local outreach initiatives targeting Medicaid and waiver populations.
  • Provide feedback on educational materials and community engagement strategies.

Geographic Scope & Travel

  • Primary responsibility for the State of Indiana, with emphasis on the Indianapolis metropolitan area during initial market expansion.
  • Frequent day travel throughout Indiana (estimated 60–75% of the workweek) is required to support partner engagement, business development, provider education, and community outreach activities.
  • This position is primarily field-based, with occasional remote administrative work for planning, reporting, and internal meetings.
  • Limited overnight travel may be required to support conferences, training, strategic initiatives, or statewide business development activities.

Required Qualifications

  • Bachelor's degree in Business, Healthcare Administration, Public Health, Marketing, Communications, or related field, or equivalent experience.
  • Minimum 3–5 years of healthcare outreach, provider relations, community engagement, or business development experience.
  • Experience working with Medicaid, LTSS, HCBS, or waiver populations preferred.
  • Excellent presentation and relationship-building skills.
  • Valid driver's license and reliable transportation. Preferred Qualifications
  • Existing relationships with Indiana case management organizations or Area Agencies on Aging.
  • Experience working with managed care organizations or community-based healthcare programs.

Key Performance Indicators

  • 25 strategic partner engagements per month.
  • 10–12 face-to-face in-services or community engagement activities per month.
  • 10 new active authorizations per month.
  • Maintain an 85% referral-to-authorization conversion rate.
  • Support retention and expansion of existing partner relationships.

Success in this role will be demonstrated by:

  • Achievement of defined KPI targets.
  • Growth in referrals and active authorizations.
  • Increased utilization among existing partners.
  • Development of strategic partnerships.
  • Positive contribution to Indiana market expansion.

Travel & Vehicle Reimbursement

This position requires frequent travel throughout the assigned territory to support relationship development, business growth, community engagement, and strategic initiatives. Employees in this role are eligible to participate in the Company's travel reimbursement program, including mileage reimbursement or a vehicle allowance, in accordance with Company policy.

Performance Incentive Opportunity

Eligible to participate in an annual performance incentive program targeted at 10–15% of base salary based on achievement of individual KPIs, strategic growth objectives, and company performance. Participation in the annual performance incentive program is contingent upon individual performance, achievement of established goals, and company performance.

Physical Requirements
