Join Our Team as a Continuity of Care Specialist!
Do you have a passion for helping individuals successfully transition from treatment to the next step in their recovery journey? We are seeking a Continuity of Care Specialist to coordinate discharge planning, connect individuals served with community resources, and help ensure the services and supports they need are in place following release.
This position plays an important role in building connections between individuals served, treatment teams, community providers, and other support systems. If you're organized, compassionate, action-oriented, and skilled at building collaborative relationships, this is an opportunity to make a meaningful difference beyond the site.
What You'll Do
• Coordinate and implement discharge planning in accordance with established policies, procedures, and individual needs.
• Complete and maintain discharge plans and related documentation to support successful transitions from the site to the community.
• Build and maintain effective relationships with treatment team members, community agencies, healthcare providers, and other internal and external stakeholders.
• Connect individuals served with community resources and services that support continuity of care following discharge.
• Facilitate applications for available financial assistance, benefits, and other resources to support individuals served after release.
• Coordinate medication, medical care, mental health services, and other healthcare needs to promote continuity of care after discharge.
• Assist individuals served with identifying and securing appropriate housing, residential placement, and other community-based supports when applicable.
• Coordinate appointments with mental health agencies and community providers to help ensure services are established prior to or following discharge.
• Provide individual and group activities focused on coping skills, life skills, community resources, and preparation for successful transition and reintegration.
• Maintain communication with community providers and follow up after discharge to support continuity of care and document relevant contacts.
• Communicate effectively with individuals served, families, treatment team members, community partners, and other stakeholders while adapting communication to meet different needs.
• Demonstrate cultural and age-specific competency, respect diverse perspectives, and build trust through professionalism, honesty, integrity, and compassionate service.
What We're Looking For
Education
• High School diploma or equivalent (Required).
• Bachelor's degree in social work or a closely related field from an accredited college or university (Preferred).
Experience
• Prior experience developing transition or discharge plans for individuals with mental illness (Preferred).
• Experience working with individuals with mental illness, serious medical conditions, or complex community support needs (Preferred).
• Experience coordinating services and resources across healthcare, behavioral health, social services, housing, or community-based organizations (Preferred).
