Job Description

Summary of position:

The Regional Team Lead assists the Executive State Director with the operations of a particular state by providing oversight concerning direction, strategic planning and supporting the coordination of services. They provide input in the coordination and improvement of services and work with provider agencies to insure timely and efficient service delivery. The position works closely with the Executive State Director to implement a shared vision, advance projects, and establish priorities within the State. Additionally, this individual will carry a modified case load of their own and make a difference by assisting people with intellectual and developmental disabilities to access and coordinate needed supports and services in community settings, and by acting as a mentor to the members of the Indiana Case Management team. Knowledge of social, economic, health or habilitative services is important. This person must have strong problem-solving skills and the ability to plan and develop individualized service plans. The Regional Team Lead must evaluate and monitor service delivery and possess the ability to provide technical assistance regarding program and client issues. An effective communicator, this person will be able to establish and maintain effective working relationships with others and to understand and apply applicable rules, regulations, policies, and procedures.

Position Requirements:

Must be currently employed with The Columbus Organization as a Team Lead in Indiana.

  • Bachelor’s Degree in Human Services from an accredited college or university.
  • At least five (5) years Case Management experience and one (1) year Case Management supervisory experience working with individuals with intellectual and developmental disabilities in Indiana.
  • Mobile Worker: This is a mobile position, meaning that regular travel to different work locations is essential. Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.
  • Reside in the same territory they are assigned to work in; exceptions may be considered, due to business need.
  • Occasional evening and weekend work may be required as job duties demand.

Essential Functions:

  • Carry a modified caseload of between 5-10 ongoing cases and complete all Indiana contract deliverables within set timeframes as identified in the Case Management service definition and BDS Quality Guide for each person assigned on caseload.
  • Establishes goals, timetables, and conducts ongoing supervisions/annual evaluations of goal achievement for each Case Manager on their respective team. Supports and encourages assigned Team Leads and Case Manager goals and objectives.
  • Monitors Team to ensure timely implementation of company policies and procedures.
  • Assists with onboarding and offboarding Case Managers. Facilitates and monitors state required system access and deactivation as needed. Works in conjunction with Columbus Manager of Organizational Effectiveness to ensure Columbus Learning Management System access is obtained and required training is assigned.
  • Facilitation of program services meetings and/or facilitates in-service training for Case Managers and assigned Team Leads. Provides oversight to ensure Case Managers and assigned Team Leads meet all training requirements.
  • Working with Team Lead to monitor caseload ratios, assigns individuals, and facilitates individual transfers to and from Columbus as well as inter-agency transitions.
  • Completes and manages JIRA tickets as needed.
  • Provides programmatic oversight for Community Waiver programs and contracts.
  • Aids the Executive State Director with monitoring adherence to contractual deliverables to ensure compliance of service delivery and accuracy of mandated reporting requirements.
  • Assists Executive State Director with analyzing program evaluations with Compliance and Quality Assurance Director to ensure adherence to the BDS Quality Guide and contract deliverables.
  • In conjunction with the Executive State Director evaluates and monitors service delivery and provides technical assistance regarding program and individual issues; determine work priorities and assigns work while ensuring proper completion of assignments.
  • Assists the Executive State Director with directing the development and implementation of annual program work plans including program goals and measurable outcomes consistent with the company’s strategic plan.
  • In collaboration with the Executive State Director mobilizes Case Managers & assigned Team Leads in the expansion of current contracts by leveraging existing customer and employee relationships.
  • Assists the Executive State Director with ensuring that assigned teams are compliant with all accrediting bodies such as CARF and that appropriate quality assurance programs are in place.
  • Reinforces an environment that fosters teamwork, cooperation, respect, diversity and service to customers.
  • Supports the Executive State Director and assists as needed to ensure service delivery per contract deliverables.
  • Comply with all rules, regulations, policies & procedures of the agency.
  • Comply with all regulatory requirements for Case Management from the BDS Quality Guide and Waiver Case Management regulations.

Knowledge/Skills/Abilities:

Ability to use Microsoft Office 365 applications.

  • Knowledge of planning principles and report preparation.
  • Skill in interpreting, policies and procedures.
  • Skill in analyzing situations, evaluating information and recommend course of action.
  • Skill in maintaining effective working relationships with fellow employees and outside agencies
  • Ability to evaluate issues, explain rules and regulations.
  • Ability to write reports and correspondence.
  • Ability to effectively present information and respond to questions from groups of managers and associates
  • Ability to define problems and collect data, establish facts and draw valid conclusions.
