POSITION SUMMARY

Provide high-quality individual, family, and group psychotherapy for individuals with mental health and/or substance use disorders. Coordinate behavioral health care within Brevard Health Alliance (BHA). Deliver evidence-based mental health services, collaborate with the multidisciplinary team in the FQHC, participate in care coordination, and support integrated primary care goals.

GENERAL EDUCATION REQUIREMENTS

Master’s degree in Social Work, Mental Health Counseling, Psychology, or a related behavioral health field from an accredited program.

ADDITIONAL QUALIFICATIONS

  • Current, active Florida license as a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC).
  • Passing score on the appropriate national clinical examination (e.g., ASWB clinical for LCSW or equivalent for LMHC).
  • Experience providing individual, family, and group psychotherapy in a community health or primary care setting; FQHC experience preferred.
  • Knowledge of integrated behavioral health practices and collaborative care models.
  • Proficient with Electronic Medical Records (EMR).
  • Excellent verbal and written communication and interdisciplinary collaboration skills.
  • Strong cultural competence and experience working with diverse, underserved populations.
  • Automated office and PC competency required.
  • Current Basic Life Support (BLS)/CPR certification from the American Heart Association or American Red Cross.
  • PRIMARY ACCOUNTABILITIES
  • Provide direct clinical services: assessment, diagnosis, and evidence-based individual, family, and group psychotherapy appropriate to patient needs and scope of practice.
  • Develop, document, and update treatment plans and progress notes in the EMR in a timely manner according to BHA policy.
  • Participate in warm handoffs, case consultations, and care planning with primary care providers, behavioral health team members, and other multidisciplinary staff to ensure integrated care.
  • Conduct mental health screenings and brief interventions in primary care visits as needed.
  • Make and coordinate appropriate referrals to community resources and specialty services; follow-up to support continuity of care.
  • Facilitate group therapy and psychoeducational groups as assigned.
  • Provide patient and family education regarding mental health diagnoses, treatment options, coping strategies, and community resources.
  • Participate in quality improvement activities, program development, chart reviews, and clinical meetings to support clinical outcomes and operational goals.
  • Maintain professional licensure, participate in continuing education, and adhere to ethical and regulatory standards.

It is the duty of each BHA employee to incorporate these goals into all daily work routines, and strive to achieve and surpass all goals by working as a team. The mission and goals of BHA will be incorporated into each associate’s Key Performance Indicators and will be evaluated on a monthly and quarterly basis.
