Purpose of the Role
The Hospice Admissions/Intake Coordinator is responsible for managing the non-clinical intake process for potential hospice patients, ensuring smooth referral coordination, and supporting the transition into hospice care. This role works closely with the Admissions/Intake team, clinical staff, and referral sources to facilitate timely and compliant patient enrollment
Key Responsibilities
- Receive and route incoming calls, emails, and referrals from patients, families, and referral sources.
- Complete all necessary paperwork, including patient verification, insurance verification, and documentation in the EHR.
- Gather and verify required documentation from referring providers.
- Coordinate with the Patient Care Coordinator to schedule admission and ensure a smooth transition into hospice services.
- Communicate with patients and families about hospice services, eligibility, and the referral process.
- Maintain a current pending list of potential patients.
- Cooperate with hospital case managers and other clinical teams to coordinate discharge plans and referrals.
- Serve as a liaison between hospice and referring hospitals, home health agencies, and community resources.
- Ensure compliance with state and federal hospice regulations, Medicare Conditions of Participation, and agency policies.
- Maintain customer relations database for referral sources.
- Perform evaluation visits as needed to assess patient appropriateness for hospice services.
- Provide empathetic, professional support to patients, families, and staff during the intake process
Required Skills & Qualifications
- Mandatory:
- Efficient, empathetic, and ethical communication.
- Strong oral and written communication skills (phone, email, in-person).
- Professional composure under pressure.
- Service-focused attitude with compassion and patience.
- Preferred / Mid-level:
- Experience with Electronic Health Records (EHR) systems.
- Organizational and multitasking abilities.
- Experience in hospice or healthcare intake processes.
- Knowledge of hospice policies, Medicare, and regulatory requirements
Education & Experience
- High school diploma or equivalent; additional education in healthcare administration or related field preferred.
- Medical background preferred, especially in hospice or home health settings
Work Environment
- Office-based with occasional travel to referral sites or patient homes.
- Fast-paced, compassionate environment requires attention to detail and strong interpersonal skills.
Compensation
- Salary: $22-24
