Position Overview:

Care Coordination services are provided to individuals referred by their Health Homes Lead or Managed Care provider as the result of one or more chronic health and/or behavioral health conditions.  Services include Intensive Home Health Care Coordination, Low Level Care Coordination, Outreach and Screening, as well as completion of a Health Action Plan to assist individuals to receive care coordinated in a manner that will increase health plan compliance, improve health outcomes and reduce hospitalization and over-utilization of specialty care or services. 

The Assistant Manager responsibilities will include maintaining on-going relationships with other community agencies that would refer to CRMHS for Care Coordination services or would provide ancillary services to CRMHS clients.  This includes individual and team meeting attendance as well as presentations to community groups including contract and governmental organizations as well as non-profit and health care clinics. 

Essential Job Functions:

  • Participation in reviewing resumes, conducting interviews 
  • Providing training, coaching, and feedback to the team 
  • Working with the Lead CC when onboarding new staff 
  • Provide Program Director and Program Lead with regular updates related to training and coaching  
  • Supervision with care coordination staff 
  • Staff evaluations 
  • Facilitate weekly/monthly team meetings 
  • Actively manage referrals from Leads 
  • Conduct weekly/monthly monitoring of project goals and outcomes 
  • Collect data for reporting and billing 
  • Participate in Tea Monitor and/or other State Audits as point person with Leads  
  • Conduct internal chart audits  
  • Attend Lead Administrative Meetings, Tier Meetings, and disseminate information in regards to feedback and program changes (some may be equally delegated to Lead) 
  • Work collectively with the Health Homes Leads to ensure that service delivery is top-notch 
  • Back up coverage to the Program Lead 
  • MOC duties weekly as assigned 
  • The Health Homes Assistant Manager will work closely with contracted Health Homes and Health Plans to ensure community based services and overall Health Homes Care Coordination is being delivered professionally and meets program guidelines, for individuals enrolled in the Health Homes Program.  
  • The Health Homes Assistant Manager will strive to carry a maximum caseload of 5-7 members, but may be a point of contact and screening for new enrollees and may provide temporary caseload coverage when another Care Coordinator is unavailable. All members will be referred to Care Coordinators for engagement and outreach.
  • Community Outreach activities require organized and targeted contact with community partners.  This includes marketing activities and managing the coordination of care.  Ability to create materials and presentations to individuals and groups regarding services available. 

Minimum Qualifications

  • Bachelor’s degree specializing in  Social Work (required).
  • Approximately 3+ years of experience in the social service field with mental illness, medically fragile and/or chemical dependency.   
  • At least 1+ years of experience in a supervisory or lead role in a social service field is required.

  • Ability to sit for extended periods while completing documentation and attending virtual or in-person meetings
  • Ability to stand, walk, and move continuously during community outreach and home visit activities
  • Occasional lifting and carrying of materials, supplies, or equipment up to 50 lbs.
  • Ability to operate a computer, keyboard, and phone for extended periods
  • Ability to drive or arrange reliable transportation to community-based appointments throughout Clark County and surrounding areas
  • Ability to navigate various environments including client homes, clinical settings, and community locations (stairs, uneven terrain, etc.
