Job Description
Position Summary:
The Outreach Liaison builds and develops relationships with community social services agencies to aid in providing education about services availability and identifying potential clients for Agape Network. The Outreach Liaison coordinates client services according to the agreements and contracts that are in place and maintains open communication with families, schools, courts and others as necessary. The Outreach Liaison meets with the Outreach Supervisor, Programs Director, and members of the Treatment Team as needed and assists in the implementation of Telehealth services.
Essential Duties and
Responsibilities:
- Act as liaison between courts, schools, families, and outside agencies.
- Develops strong partnerships/relationships with potential referral sources through active presentations, meetings and daily/weekly client recruitment efforts utilizing an ability to relate to people in a manner conductive to winning their confidence and establishing their support.
- Responsible for planning and coordinating referral/outreach activities with schools, and other community agencies, and related admissions procedures.
- Manage contact information, follow up with clients, and keep relationships strong/active.
- Assists clients/families with access to recommended services.
- Share information with clinical, and admissions department, as needed.
- Completes documentation and reports as necessary.
- Discuss new ideas and ways to reach organizations with Management.
- May be required to attend community events at night and on weekends.
- Provides on-call services as needed.
- Attends staff meetings and trainings as required.
- Upholds total confidentiality with respect to clients in all instances.
- Assist with the implementation of Telehealth Services.
- Turn in timesheets weekly according to billing requirements.
- Perform other related duties as assigned by Outreach Supervisor and/or Programs Director
Education and/
or Experience: High school degree or equivalent experience in a related field. Bachelor’s degree preferred. Must be able to provide their own vehicle, vehicle insurance and have current FL driver’s license in case of emergency.
