Patient Care Representative

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Patient Care Representative based in Canada.

This fully remote role supports patients throughout their healthcare journey, combining compassionate service with accurate, clinically appropriate communication.
You’ll help patients navigate intake, scheduling, education, triage, and ongoing treatment support in a patient-centered environment.
The role requires strong listening and communication skills, allowing you to understand patient needs and provide clear guidance aligned with established clinical protocols.
You’ll manage both inbound and outbound interactions while maintaining detailed and compliant patient records.

Accountabilities
  • Support patient intake and coordination across core functions such as education, triage, and appointment booking, ensuring a clinically appropriate and patient-centered experience.
  • Schedule new patient appointments, follow-ups, and rescheduled visits while supporting efficient clinic flow and provider availability.
  • Conduct proactive outbound outreach to encourage appointment adherence, continuity of care, and completion of required treatment steps.
  • Respond to patient questions and concerns, gather relevant patient history, and provide appropriate recommendations within established guidelines.
  • Deliver clear, high-quality medical cannabis education through digital platforms, telephone, or clinic interactions, helping patients understand how to use and order cannabis as part of their treatment.
  • Maintain and continuously improve patient messaging, educational content, and knowledge standards in collaboration with the wider Patient Care Team.
  • Provide compassionate and empathetic patient support while communicating confidently, directly, and professionally.
  • Manage inbound and outbound communications with patients and other stakeholders through phone and email, ensuring timely and appropriate responses.
  • Perform structured patient triage by assessing needs, eligibility, and urgency during patient interactions.
  • Accurately document patient interactions in EMR systems and supporting tracking tools while maintaining privacy, completeness, and internal compliance requirements.
  • Support patient-facing events and clinic initiatives through scheduling, outreach, and follow-up communications designed to strengthen engagement and attendance.
  • Consistently meet or exceed established KPIs and performance objectives while contributing to process improvements and other team initiatives as required.
Requirements
  • 3+ years of relevant professional experience is preferred.
  • Customer service or call-center experience, particularly within healthcare or hospitality environments, is an asset.
  • Experience working with clinical databases or EMR systems is preferred but not mandatory.
  • Strong written and verbal English communication skills are required; French fluency is an asset, with bilingual English/French candidates particularly valued.
  • Strong critical-thinking and problem-solving abilities, with sound judgment when handling challenging or sensitive patient situations.
  • Demonstrated empathy, emotional intelligence, active listening, and a genuine passion for helping people improve their health.
  • Meticulous, proactive, organized, and capable of contributing to a positive and collaborative work environment.
  • Strong analytical and thought-leadership skills, with the ability to identify opportunities that improve productivity and operational efficiency.
  • Ability to thrive in a fast-paced, evolving environment and adapt positively to changing priorities, schedules, processes, and responsibilities.
  • Strong collaboration, communication, and leadership skills, with the confidence to share ideas and help turn them into practical improvements.
  • Ability to work independently while maintaining high standards of patient care, documentation, confidentiality, and professionalism.

  • English-stream compensation: $20.19 CAD per hour.
