Job Description
Position Summary
The Population Health Planner uses outreach reports and practice registries to proactively identify patients due or over-due for certain health services, contacts the patients, activates/motivates the patient to complete the recommendation, and assists in scheduling the patient as needed. Prioritization of projects will occur in consultation with the practice management.
Primary Position Responsibilities
1. Preventative Care and Chronic Conditions Outreach based on patient registries and reports
2. Scheduling services at the practice and assisting with services provided external to the practice via Centralized Scheduling
3. Assists in closing the loop on incomplete orders
4. Meeting Attendance, Practice Team Meetings, PHP meeting and meeting as assigned by role
