Summary:
Vighter is recruiting Registered Nurse (RN) Case Manager-Disease Management to provide patient care and administrative support at Joint Base Andrews, Maryland and Joint Base Anacostia-Bolling, Washington, DC. In accordance with established policies, procedures, protocols, and guidelines, while supporting the healthcare needs of Service Members, their families, and Veterans.

Principal Accountabilities:
- Coordinate patient care across the continuum of care and support quality, clinical, and cost-effective outcomes.
- Identify patients at risk for chronic diseases and co-morbid conditions.
- Assess physical, psychosocial, and overall health needs and develop appropriate treatment plans.
- Provide disease-management education, interventions, and patient training.
- Conduct in-person, telephone, and secure-message patient evaluations.
- Coordinate with Case Managers, Utilization Managers, providers, and interdisciplinary teams.
- Track patient outcomes and maintain the Disease Management registry.
- Provide clinical procedures and patient education within the RN scope of practice.
- Recognize and respond appropriately to emergency conditions.
- Document assessments, interventions, outcomes, tracking logs, and required reports.
- Participate in staff education, meetings, committees, and quality-improvement activities.
- Follow HIPAA, DoD, DHA, MTF, The Joint Commission and professional nursing standards.
Knowledge and Skills:
- Strong knowledge of chronic disease management, population health, preventive care.
- Ability to assess patients in person and telephonically.
- Knowledge of medications, patient education, and clinical nursing practices.
- Proficient with Microsoft Office, Outlook, internet applications, and electronic medical records.
- Strong communication, critical-thinking, and problem-solving skills.
- Ability to communicate effectively with patients, families, and healthcare teams.
Education:
- Associate Degree in Nursing (ADN) from an ACEN- or CCNE-accredited nursing program.
Qualifications:
- Minimum of three (3) years of full-time outpatient nursing experience.
- Minimum of two (2) years of experience managing and educating patients with chronic diseases, including diabetes, hyperlipidemia, and hypertension.
- Current, active, and unrestricted Registered Nurse license.
- Current Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) are both required.
- Disease-management certification such as CCP, CHC, CDE, or AE-C.
- Successful completion of the 5-day Disease Management Course, if required by the MTF.
- Recent experience with electronic medical records, data entry, and computer systems.
Duty Hours / On-call Requirements:
- The expected duty hours for this position are 8 hours per day and 5 days per week. However, this position supports 24/7 operations which may require work to be conducted at night and on weekends.

Travel:
- As required to satisfy the contractual obligations. Commuting is non-compensatory. All official travel is government funded.

Physical Requirements:
- Must be able to stand or sit for more than 8 hours per day.
- Must be able to use an elevator or walk up & down stairs.
- Must be able to lift up to 40 pounds and carry up to 10 feet.
- Must be able to routinely grasp or handle objects, use finger dexterity, bend elbows/knees and reach above and below shoulders.
- Must be able to read and interpret handwritten and typewritten print.
- Must be able to communicate by voice and detect sound by ear.
- Must be able to perform duties in an indoor office or outdoor environment.
- Must be able to pass a criminal background investigation.
- Must undergo Urine Drug Screening (UDS).
- Must be a Citizen of the United States or legally authorized to work in the United States of America.
