Job Description

CPLC LUCES (Latinos Unidos Contra el Sida/ Latinos United Against AIDS) provides HIV prevention education and supportive services to people living with or at risk of HIV/AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.

POSITION SUMMARY

Under general supervision, provides direct covered services and treatment to individual clients, coordinates with community support and stakeholders. Responsible for managing appointment schedule as well as conducting targeted outreach to people with a known HIV disease or at high risk of HIV transmission so they may be enrolled in care and treatment services.

MINIMUM QUALIFICATIONS AND COMPETENCIES

  • Bachelor’s degree from an accredited college in a field related to case management such as social work, nursing, public health, or other human services related field preferred.
  • Or high school diploma and two years of comparable professional knowledge, skills and abilities that documents experience with working with HIV positive or high-risk individuals.
  • Knowledge of HIV/AIDS and skilled at educating clients about HIV/AIDS and other STI’s.
  • Able to work a flexible schedule weekdays and weekends as needed per program requirements.
  • Bilingual: English/Spanish skills. Must be able to communicate to Spanish speaking clients, considering the importance of knowing & communicating medical terminology in both languages.
  • Must have access to reliable, safe transportation and be willing to travel throughout Maricopa County and Pinal County. This position requires driving on behalf of the company. Candidates must hold a valid driver’s license, meet the company’s motor vehicle record (MVR) standards, meets company’s insurance eligibility, and comply with all state and federal driving requirements. Ongoing employment is contingent upon maintaining an acceptable driving record.
  • Ability to work a flexible schedule on weekdays and weekends as needed per program requirements.

  • Proven ability to provide non-judgmental, unconditional support to clients.
  • Proven organization, communication, and time management skills.
  • Possess good written and verbal communication skills.
  • Ability to communicate and coordinate services with other treatment providers.
  • Ability to attend and actively participate in team staffing.
  • Ability to effectively engage clients and in some cases, their family members in treatment.
  • Experience working in different settings with clients including but not limited to in-office, community-based, clinic-based and in-home service delivery.
  • Must successfully complete the agency’s orientation and demonstrate competency regarding skills and knowledge of HIV prevention.
  • Pre-Employment Requirements: Employment in this position is contingent upon successful completion of all required pre‑employment screenings and subject to credentialing checks to maintain compliance with licensing and contractual requirements.

  • Fingerprint-based background check in accordance with federal, state, and contract requirements.
  • Central Registry Check: Employment in this position is contingent upon passing a Central Registry check, as required by state regulations or program standards for working with vulnerable populations
  • Criminal history background check meeting applicable state, federal, licensing, or program standards.
  • Candidates may also be required to complete additional screenings or provide documentation as necessary to comply with funding, licensing, contractual, or regulatory obligations.
