Job Description
JOB SUMMARY
The Outreach and Enrollment Manager oversees the outreach and program enrollment. He or She assists the Chief Executive Officer, Chief Administrative Officer, Chief Development Officer, and Veterans Program Director with outreach, events, enrollment, and community involvement.
ESSENTIAL JOB FUNCTIONS
• Serves as a liaison between veterans, communities, and FSC
• Provides guidance and social assistance to veterans and community residents
• Identifies and helps enroll eligible individuals in federal, state, and local private or nonprofit health and human services programs
• Provides information on available resources
• Assists with client access to community and governmental based service agencies including help with completing applications and registration forms, as well as overcoming financial / transportation barriers to obtaining recommended care for domestic violence, housing needs, food insecurity, unemployment, etc.
• Provides referrals to community resources
• Provides public health education, follow-up and outreach
• Advocates for individual and community health
• Identifies and assists vulnerable individuals and/or individuals with complex and unmet health needs in underserved populations who are not yet connected to the health care system
• Conducts eligibility determination, enrollment and follow-up with uninsured clients
• Educates and encourages individuals and communities with adopting healthy behaviors
• Promotes preventative care and screenings
• Attends seminars/workshops, actively participates in professional organizations
• Maintains certification or licensure
• Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area.
• Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans.
MARGINAL OR PERIODIC JOB FUNCTIONS
? Performs related duties as required
REQUIRED EDUCATION / EXPERIENCE
? Bachelor's degree or certification in Community Health.
? Experience preferably with a healthcare or community service orientation.
? Knowledge of grant application process, scoring criteria and funding cycles.
? Proven record of securing new funding opportunities including the ability to distinguish and identify opportunities for FSC across a broad span of community needs.
? Excellent Computer skills (Microsoft Office Word, Access, PowerPoint, and Excel), and database management skills.
? Previous grant budget development and management experience.
KNOWLEDGE/SKILLS/ABILITIES
Strong social and interpersonal skills. Community involvement. Effective written and verbal communication skills. Ability to work cooperatively with other professionals either in teams or in consultation. Knowledge of, and compliance with, the principles of professional ethics. Knowledge of computers, software applications, and mobile communication devices.
Bi-lingual/Spanish-speaking a plus
