LOCATION: Sylvania, GA and Millen, GA

Job Description/Duties:

  • Providing a broad range of social services to individuals served in Screven and Jenkins County Schools and their families.
  • Ensuring individuals go to their scheduled doctor appointments, take medications as prescribed.
  • Having contact with consumers parents, teachers, and school counselors on a weekly basis and reporting findings to supervisor and MD/reflect in progress notes.
  • Completing progress notes, no later than 24 hours after delivering the service, having said progress notes scanned and shown in the EMR no later than 7 days after providing the services, and checking EMR weekly to ensure that progress notes are showing in EMR.
  • Providing billable hours to average 5.8 hours per day to be measured by daily productivity spreadsheets maintaining at least 65%.
  • Working with scheduler in completing weekly schedule to ensure monthly contacts are met and maintained.
  • Communicating regularly with staff on progress toward defined goals and/or required results, providing specific feedback, and initiating corrective action when defined goals and/or required results are not met. Conferring regularly with staff to review employee relations climate, specific problem areas, and actions necessary for improvement.

This position requires frequent travel to and from the individuals' schools and homes.

Requirements:

  • High School Diploma or equivalent education required, but bachelor's degree preferred.
  • Valid GA Driver's License, ability to travel frequently in company car.
  • Pass a drug screen and background check
  • General Knowledge of Microsoft Programs such as Outlook and Excel
    • Must be comfortable learning how to use new computer software.
