JOB OVERVIEW

Since 2016 Midway Living, Inc. ("Midway") has been a catalyst for transformative change in the lives of homeless families, empowering them to discover their inherent strengths and to chart a path out of homelessness, allowing them to redefine their narratives and lead lives of purpose.

Under general supervision, the case manager supervisor plays a pivotal role in ensuring the smooth day-to-day operation and coordination of program-related staff and services. This position is responsible for overseeing and supporting the case management team to achieve program goals, targets, and performance outcomes effectively and efficiently. The role may require availability during early evening hours and occasional weekends.

ESSENTIAL JOB FUNCTIONS

  • Directly supervise case management staff, providing ongoing guidance, coaching, and performance management.
  • Maintain effective communication with clients, staff, and peers to ensure smooth program operations.
  • Uphold professional relationships with clients and maintain strict confidentiality.
  • Plan, organize, and optimize program activities to meet contractual goals and performance targets.
  • Develop and implement program content in collaboration with staff and management.
  • Troubleshoot client and staff program issues, making decisions aligned with program policies and procedures.
  • Oversee and improve staff performance through feedback, training, and regular supervision sessions.
  • Address resistance to change among clients, staff, and supervisors/funders effectively.
  • Administer constructive feedback, in accordance with organizational policies.
  • Monitor clients' progress toward goals, documenting achievements and milestones.
  • Review and ensure accuracy, completeness, and clarity of all client progress documentation.
  • Review, correct, and sign service plans and recertifications on a regular basis.
  • Conduct regular chart audits of client files to ensure compliance and quality of services.
  • Add supervisory notes to CARES and Midway Living portals.
  • Lead and participate in administrative and staff meetings as required.
  • Provide necessary information for weekly, monthly, quarterly, semester, and annual reports to management.
  • Regularly update progress on the Midway Living portal.
  • Perform other related tasks as requested.

EDUCATION & EXPERIENCE

  • Master’s degree in social work and a minimum of three years’ experience
  • Knowledge of Homeless Population
  • Compatible Philosophy with Midway Living

QUALIFICATIONS & PREFERRED SKILLS

  • Strong interpersonal skills
  • Excellent Oral and Written Communication Skills
  • Effective organizer and communicator
  • Proficient in Microsoft Office Suite
  • Ability to adapt to new software applications
  • Capable of working independently or collaboratively
