Discharge Planner at AnMed – Anderson, South Carolina

Provides discharge planning, financial counseling, psychosocial evaluations, and grief therapy for patients/families as indicated. Remains knowledgeable of community resources so as to refer patients appropriately. The social worker provides these services to patients from prenatal to geriatrics in both inpatient and outpatient settings.

Qualifications:

• Bachelor's degree in social work, psychology, or sociology required.
• One to two years of experience in social services or hospital discharge planning preferred.
• Knowledgeable regarding community resources.
• Excellent communication skills.
• Works in collaborative team approach.

Preferences:

Master's degree in Social Work.
