BASIC PURPOSE OF THE JOB

Licensed pharmacist with a working knowledge of pharmacy and clinical applications related to pharmacy. The pharmacist dispenses medications and pharmaceutical preparations as prescribed by the provider. The pharmacist maintains proper records (i.e. computerized patient profiles) and supplies information regarding pharmaceuticals to hospital staff and patients. Evaluates medication orders for appropriate medication use. Performs advanced pharmaceutical work. Work involves directing, overseeing, and organization pharmaceutical programs, activities, or operations.

  • Director, Pharmacy; Manager, Pharmacy; Coordinator, Pharmacist
JOB REQUIREMENTS

Supervisory Responsibilities: YES; Pharmacy Technicians/Interns/Students

Minimum Education: Bachelor’s Required in Pharmacy; Doctor of Pharmacy Preferred

License/Certification Required: Pharmacy license in the State of Tennessee, Relevant board certification required

Minimum Work Experience: 4 years of relevant pediatric experience required

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
  • Strong communication, mathematical, and computer skills.
DUTIES AND RESPONSIBILITIES
  • Provides collaborative pharmacy practice services including primary healthcare, consultative services and drug information services to patients and other healthcare providers as applicable.
  • Initiates quality improvement and pharmacy education including: drug utilization review, adverse drug reactions, in-services for health professionals, and pharmacist education
  • Guides, oversees, assesses, and reviews the work of assigned pharmacists, interns, and pharmacy students for productivity, efficiency, and quality.
  • Oversees and/or conducts research and analyses of pharmaceutical policy issues and problems and the economic effect of drug utilization.
  • Plans and directs investigative and inspection programs as applicable.
  • Researches and evaluates the clinical efficacy and safety of drug products.
  • Provide pharmaceutical informatics support as it relates to pharmacy services.
  • Oversee specialized service lines within the department of pharmacy as it relates to defined practice areas (e.g. regulatory compliance, medication safety, Neonatal Intensive Care, Hematology/Oncology, etc.). Defined by department director in conjunction with pharmacy leadership.
  • Responsible for the preparation and distribution of medication orders filled for the inpatient, outpatient, and ancillary areas. Oversees the acquisition and disbursement of drugs and medications to various offices, clinics, and other sites.
  • Works collaboratively and cooperatively with pharmacy, nursing, administration, and medical staff to ensure optimization of medication use process.
  • Supervises pharmacy interns and technicians in duties related to their professional practice.
  • Dispenses medication for patient care to hospital staff and patients in appropriate forms. Determines appropriateness of dosage form for the size and weight of the child; contacts physician if in doubt of appropriateness of dosage.
  • Appropriately prioritizes medication orders.
  • Appropriate use of pharmacy automation including discrepancy resolution & billing adjustments.
  • Demonstrates a clean and organized workflow
  • When assigned to a patient care service, communicates with service area staff regarding patient care and medication therapy issues and decision points.
  • Documents interventions completed during patient care service.
  • Serves as the liaison between the assigned patient care service area and the central pharmacy staff on patient care and drug distribution issues.
  • Demonstrates proficiency with math calculations, including calculating appropriate doses for different age groups or diagnoses.
  • Prepares accurate labels utilizing the proper labeling format.
  • Prepares/Assesses age-appropriate medication for compounding, filling, and dispensing.
  • Takes appropriate steps to prevent clinical dispensing errors.
  • Clinical Pharmacy Assessments (electrolytes, lab data, kinetics, etc.)
  • Performs pharmacokinetic services including: drug level interpretation, drug dosing and scheduling recommendations, nursing and pharmacy education
  • Performs pharmacy related consults based on provider orders.
  • Maintains accurate and updated records.
  • Maintains medication stock and stays up to date regarding backorders and shortages.
  • Ensures pharmacy equipment is operational and maintenance is performed regularly before use.
  • Performs compounded non-sterile and sterile preparations when applicable.
  • Follows all current policies and procedures of ETCH, Pharmacy, and State and Federal laws.
  • Reports unusual occurrences with inventory to the manager or lead pharmacist in their absence.
  • Administers additional pharmacy services including but not limited to: immunizations, wellness screenings, medication therapy management and specialty medication management if applicable.
  • Serves as active member of Pharmacy and Therapeutics as well as other committees as assigned.
  • Performs miscellaneous job-related duties as assigned.
PHYSICAL REQUIREMENTS
  • Note: Reasonable accommodations may be made for individuals with disabilities to perform the essential functions of this position.
  • Light lifting, pushing and pulling is required for 10-20 lbs occasionally and frequent moving of objects of less than 10 lbs is required. Frequent sitting with some walking, standing, squatting, bending and reaching is required. Keyboard/computer use and/or repetitive motions may be required.
