Job Description
ESSENTIAL DUTIES:
1. Coordinate patient identification, sourcing, and mobilization activities across
outreach locations and mobile clinics.
2. Coordinate the full patient care journey, including pre-admission, admission,
treatment, discharge, and post-care follow-up processes.
3. Coordinate the patient pipeline from initial identification to treatment completion,
tracking conversions from referral to care delivery.
4. Implement community awareness and mobilization activities and strengthen
community partnerships and referral networks.
5. Coordinate patient transport logistics under CURE programs and implement
community initiatives under different programs
6. Maintain robust patient communication and feedback systems to support patient
experience and satisfaction initiatives.
7. Assisting in Home Visits for Patients Follow Up and documentation
8. Conduct Social Work-related activities in Patient Mobilization, Recruitment and
Follow-up that will support CURE activities and programs.
9. Coordinate regularly with Business Development, Programs, Clinical, and
Booking teams to optimize patient scheduling and care pathways.
10. Generate patient mobilization, referral tracking, and conversion reports to guide
operational decisions.
11. Actively support the achievement of overall hospital patient growth targets.
