Affinity Healthcare Solutions Skilled Care is seeking compassionate and professional Physical Therapists to provide high-quality home health services throughout Philadelphia, Montgomery, Delaware, Chester, Bucks, and Lehigh Counties.

As a Physical Therapist, you will provide personalized care in patients' homes, helping them improve mobility, regain independence, and enhance their overall quality of life. The ideal candidate will have strong clinical skills, excellent documentation abilities, and previous Home Health experience.

Responsibilities

- Evaluate patients and develop individualized treatment plans
- Provide physical therapy services in the home setting
- Help patients improve mobility, strength, balance, and functional independence
- Incorporate home environments into treatment plans to optimize therapy outcomes
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
- Educate patients and caregivers on exercises, safety techniques, equipment use, and preventative measures
- Monitor patient progress and modify treatment plans as needed
- Maintain accurate and timely clinical documentation
- Complete all patient notes and communications within 24 hours
- Ensure compliance with agency, state, and federal regulations
- Travel to patient homes throughout assigned coverage areas

Qualifications

- Active Pennsylvania Physical Therapist (PT) license
- Minimum of 2 years of Physical Therapy experience preferred
- Graduate of an accredited Physical Therapy program
- Home Health experience preferred
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal skills
- Strong computer and documentation skills
- Reliable transportation and valid driver's license
- Ability to work independently and manage a flexible schedule
- Knowledge of electronic medical record systems is a plus
