Our nonprofit organization is currently seeking a Full-Time Assisted Living Care Coordinator to join our team! A compassionate, skilled Care Coordinator to support the behavioral health and emotional well-being of older adults. This role connects residents with needed resources and support, provides behavioral health interventions, and offers crisis support when appropriate. The ideal candidate has experience supporting older adults and individuals with behavioral health needs, including depression, anxiety, grief and loss, adjustment challenges, cognitive changes, behavioral concerns, and other psychosocial issues. The Care Coordinator serves as an essential behavioral health resource within our senior living community, promoting dignity, connection, emotional well-being, and quality of life for residents and their families.

Schedule: Monday-Friday, 40 hours per week, between 7:30am and 5:30pm

Essential Duties and Responsibilities: Other duties may be assigned.

•Provide short-term behavioral health, psychosocial assessment, consultation, advocacy, and supportive services to assisted living residents and their families while helping them transition to longer-term supports that promote emotional well-being, independence, dignity, housing stability, and quality of life.

•Conduct and document behavioral health, psychosocial, cognitive, functional, and Health & Wellness assessments, including annual Health & Wellness Discussions, SLUMS screenings, microwave evaluations, risk assessments, and other required evaluations to identify resident strengths, needs, goals, and support plan requirements.

•Monitor resident well-being, including changes in mood, behavior, functioning, hospitalizations, temporary placements, and community returns, implementing appropriate interventions and follow-up as needed.

•Coordinate and facilitate resident care plan reviews in collaboration with the Wellness Coordinator, ensuring care plans accurately reflect changing resident needs and service requirements.

•Collaborate with residents, families, caregivers, healthcare providers, case managers, staff, and interdisciplinary teams to address behavioral health, psychosocial, medical, and housing-related concerns.

•Assist residents and families with transitions related to housing, healthcare needs, cognitive decline, loss of independence, end-of-life concerns, and community resource navigation, including placement when residents no longer meet assisted living eligibility requirements.

•Coordinate referrals to behavioral health, psychiatric, counseling, community support, housing, and other service providers as appropriate.

•Assist residents with applications and access to benefits and services, including Medicaid, SSDI, AND, HCBS, PTC, long-term care programs, and other available resources when needed.

•Manage and coordinate Medicaid renewals and eligibility-related issues, working directly with government and community agencies to ensure continuity of benefits and services.

•Serve as a liaison and advocate regarding insurance benefits, Medicaid programs, long-term care options, and other resident support services.

•Coordinate with Health & Wellness staff regarding resident appointments, activity participation, hospitalizations, temporary placements, and other care coordination needs.

•Provide consultation, education, training, and support to staff and families regarding mental health, behavioral symptoms, communication strategies, aging-related psychosocial issues, bereavement, and resident support needs.

•Facilitate therapeutic, educational, support, and engagement groups and programs that promote social connection, community involvement, and resident well-being.

•Assist residents with practical support needs, including correspondence, reading mail, telephone communication, and other activities that promote independence and self-advocacy.

•Maintain timely, accurate, objective, and confidential documentation of assessments, interventions, resident interactions, referrals, incidents, care coordination activities, outcomes, and required program data in PointClickCare and other required systems.

•Provide regular communication and updates to authorized family members, guardians, powers of attorney, case workers, and other approved representatives regarding resident status, care needs, and service coordination.

•Establish and maintain collaborative relationships with community agencies, healthcare partners, and service providers to enhance resident support and resource availability.

•Track outcomes, program metrics, and quality indicators to support compliance, quality improvement, and service effectiveness.

•Support Health & Wellness Department operations, staffing, and program initiatives as needed.

•Perform other duties as assigned.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field required.
  • Master's degree in Social Work or a related behavioral health field preferred.
  • Experience working with older adults/aging populations required.
  • Strong understanding of the psychosocial and behavioral health needs of older adults.
  • Excellent clinical judgment, communication, documentation, and interdisciplinary collaboration skills.

Preferred Experience

  • LCSW or other advanced clinical social work licensure.
  • Experience in senior living, assisted living, skilled nursing, memory care, hospice, or geriatric behavioral health.
  • Experience with individual counseling, behavioral health interventions, crisis intervention, or psychosocial assessment.
  • Experience supporting individuals with dementia, depression, anxiety, grief, or other behavioral health concerns.
