Principal Engineer, Applied AI (Fullstack/Backend)

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Principal Engineer, Applied AI (Fullstack/Backend) based in the United States.

This is a high-impact technical leadership role focused on transforming AI experimentation into reliable, production-grade experiences.
You will architect the services and systems that power patient-facing experiences from initial intake through ongoing digital care interactions.
Working at the intersection of applied AI, backend engineering, and product development, you will turn promising ideas into scalable solutions.
You will establish engineering standards around reliability, observability, debugging, performance, and operational readiness from the outset.
The role combines hands-on architecture and coding with organization-wide technical influence and mentorship of senior engineering talent.
You will work closely with research, product, and clinical stakeholders while navigating ambiguity and accelerating strategic initiatives.

Accountabilities
  • Own the overall architecture of the Applied AI technology stack, designing how services integrate into a cohesive, end-to-end patient-facing experience.
  • Architect and evolve scalable backend and fullstack services supporting AI-powered experiences from intake through ongoing customer interactions.
  • Partner with research and experimentation teams to productionize emerging AI capabilities, introducing the engineering rigor required for reliability, uptime, maintainability, and scale.
  • Establish strong practices for observability, debugging, monitoring, resilience, and operational readiness across production systems.
  • Design systems and team boundaries that enable sustainable technical and organizational growth while addressing non-functional priorities early.
  • Balance near-term product delivery with long-term architectural durability, scalability, system health, and customer experience.
  • Translate customer and product needs into resilient technical architectures that anticipate clinical complexity and future growth.
  • Influence technical strategy and lead cross-organizational engineering initiatives, creating alignment and momentum across teams.
  • Navigate ambiguity by clarifying ownership, resolving blockers, simplifying decisions, and accelerating execution of strategic initiatives.
  • Mentor senior technical leaders and engineers, strengthening technical standards, talent development, feedback, and continuous learning.
  • Champion customer-centric engineering principles and architectural patterns that embed customer needs into the platform.
  • Identify and proactively address scalability constraints, migration needs, reliability gaps, and organizational dependencies before they become critical issues.
Requirements
  • 15+ years of professional software engineering experience, with deep architectural expertise and a strong record of delivering production systems at scale.
  • Proven experience influencing technical strategy and delivering complex, cross-organizational engineering initiatives at a group or enterprise level.
  • Extensive experience designing, launching, and operating highly scalable production services with a focus on robustness, reliability, observability, and debuggability.
  • Deep understanding of full-stack service architecture and how multiple services should work together to create cohesive product experiences.
  • Strong hands-on engineering capabilities and a desire to remain close to code, architecture, and technical implementation rather than operating solely at the roadmap level.
  • A demonstrated productionization mindset, particularly the ability to bring engineering discipline and operational excellence to fast-moving research and experimentation environments.
  • Exposure to LLMs, AI services, or AI-powered applications, with an understanding of how these technologies can be integrated into production software systems.
  • Ability to make thoughtful architectural decisions that balance immediate business needs with long-term scalability and sustainability.
  • Strong communication and collaboration skills, with the ability to work effectively across engineering, research, product, and clinical functions.
  • Demonstrated ability to create alignment and momentum in ambiguous or complex technical environments.
  • Strong customer orientation and the ability to translate customer needs into scalable, resilient backend and platform solutions.
  • Proven mentoring and leadership capabilities, particularly in developing senior engineering talent and reinforcing high technical standards.
  • Curiosity, adaptability, humility, transparency, and sound judgment when making decisions in rapidly evolving environments.
  • A proactive, action-oriented approach to identifying blockers, resolving dependencies, and improving execution without requiring perfect clarity.
  • Interest in helping define how AI can reshape both technology infrastructure and customer-facing products from the ground up.
