We are seeking a dependable, patient-focused Medical Assistant (MA) / Care Manager to support daily clinic operations while helping patients stay connected to their care plans between visits. This position combines hands-on clinical support with ongoing patient outreach, chronic care management, and coordination of services.

The ideal candidate is organized, compassionate, detail-oriented, and comfortable balancing in-office clinical responsibilities with patient follow-up and care coordination.

Position Summary

The Medical Assistant / Care Manager assists providers during patient visits and supports ongoing chronic care management under appropriate supervision. This role helps coordinate appointments, referrals, medications, and follow-up services while identifying barriers that may prevent patients from following their care plans.

All duties must be performed within the employee's training, credentials, authorized scope, and applicable New Jersey requirements. Clinical concerns requiring assessment, diagnosis, treatment decisions, or changes to a care plan must be referred to an appropriately licensed clinician.

What You'll Do

Clinical Support

  • Prepare examination rooms, room patients, and obtain and document vital signs, symptoms, medical histories, allergies, and current medications.
  • Assist providers with examinations and procedures within the employee's training and authorized scope.
  • Perform specimen collection, phlebotomy, injections, and vaccinations when appropriately trained, authorized, and permitted under applicable requirements.
  • Maintain clean, organized, and properly stocked examination rooms.
  • Follow infection-control procedures and established patient safety protocols.
  • Coordinate follow-up appointments, diagnostic testing, and referrals ordered by providers.
Care Management & Patient Follow-Up

  • Help identify eligible patients for chronic care management (CCM) and support outreach, enrollment, and consent documentation using approved workflows.
  • Conduct scheduled patient check-ins to gather updates regarding appointments, medications, patient-reported concerns, and progress with existing care plans.
  • Reinforce provider-directed instructions, including appointment follow-through, preventive care, and medication adherence.
  • Identify barriers to care, such as transportation, medication access, cost, or difficulty scheduling services, and help coordinate appropriate resources.
  • Support transitions of care following hospitalizations or emergency department visits by arranging follow-up and communicating outstanding needs to the care team.
  • Escalate reported symptoms, medication discrepancies, and clinical questions to the appropriate licensed clinician.
Medication & Care Coordination

  • Collect and document patient-reported medication information and flag discrepancies for provider or pharmacist review.
  • Assist with medication refill requests, prior authorization paperwork, pharmacy communications, and medication delivery coordination.
  • Track referrals, diagnostic testing, appointments, and outstanding patient needs through completion.
  • Follow up with patients and service providers to help minimize delays and support continuity of care.
Documentation & Team Communication

  • Maintain accurate, timely electronic health record (EHR) documentation of patient interactions, outreach attempts, care coordination activities, and follow-up needs.
  • Record eligible chronic care management activities and time in accordance with applicable program, documentation, and billing requirements.
  • Communicate effectively with providers, pharmacies, and other care-team members.
  • Protect confidential patient information and follow HIPAA requirements, organizational policies, and quality standards.
  • Participate in required training, competency assessments, and ongoing workflow improvements.
What You Bring

Required Qualifications

  • Medical Assistant education, training, or relevant clinical experience.
  • Ability to obtain vital signs, document patient information, and work accurately within an electronic health record system.
  • Strong communication, organization, and follow-through skills.
  • Ability to communicate professionally with patients, providers, pharmacies, and other care-team members.
  • Ability to balance in-office clinical responsibilities with ongoing patient outreach and care coordination.
  • Willingness to complete EPIC Moodle care-management training and required competency assessments.
  • Successful completion of applicable background checks and pre-employment requirements.
Preferred Qualifications

  • Previous experience as a Medical Assistant in a primary care, outpatient, or multi-specialty healthcare setting.
  • Experience with patient outreach, chronic care management, care coordination, or population health.
  • Phlebotomy experience.
  • Familiarity with medication reconciliation, referrals, prior authorizations, and transitions of care.
  • Experience using EHR systems and documenting chronic care management activities.
Scope of Practice & Supervision

This position supports provider-directed care and does not independently diagnose conditions, conduct clinical triage, recommend medication changes, or modify treatment plans.

Clinical concerns, symptoms requiring assessment, medication discrepancies, and requests for changes to care must be referred to an appropriately licensed clinician. All assigned clinical duties must be consistent with the employee's training, credentials, authorized scope, and applicable New Jersey laws and regulations.
